What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Jul 18, 2024: standard inspection1 deficiency
  • Ic - Clean/Dirty Areas, Med Prep Area, No Common Med Carts

    The inspectors’ words, 425 words

“IC-CLEAN/DIRTY;MED PREP AREA;NO COMMON CARTS CFR(s): 494.30(a)(1)(i) Clean areas should be clearly designated for the preparation, handling and storage of medications and unused supplies and equipment. Clean areas should be clearly separated from contaminated areas where used supplies and equipment are handled. Do not handle and store medications or clean supplies in the same or an adjacent area to that where used equipment or blood samples are handled. When multiple dose medication vials are used (including vials containing diluents), prepare individual patient doses in a clean (centralized) area away from dialysis stations and deliver separately to each patient. Do not carry multiple dose medication vials from station to station. Do not use common medication carts to deliver medications to patients. If trays are used to deliver medications to individual patients, they must be cleaned between patients. This Standard is not met as evidenced by: 38766 Based on policy review, observations, and staff interviews, the facility staff failed to ensure a cart, containing clean supplies, was not located next to a handwashing sink/eyewash station in 1 (one) of 1 (one) observation. Findings included: A review on 07/16/2024 of the facility policy “Dialysis Precautions” published 01/04/2012 revealed “ ...Clean Versus Dirty Areas Clean area: An area designated for clean and unused equipment, supplies and medications. Dirty area: An area where this is a potential for contamination with blood or body fluids and areas where contaminated or used supplies, equipment, blood supplies, or biohazard containers are stored or handled... Clean areas should be separated from dirty areas where used supplies, equipment, or blood samples are stored or handled...” Observation on 07/16/2024 at 1100 of the treatment area revealed a handwashing sink also used as an eyewash station across from station #2. The observation showed a clean supply cart was near the left side of the sink. The observation failed to reveal a plexiglass divider on the left side of the sink to prevent water from splashing onto the clean supplies located on the top shelf of the cart. The observation revealed clean supplies were in the immediate area of a handwashing sink and eyewash station. An interview on 07/17/2024 at 1230 with the Biomedical Technician and the ATOM revealed that the facility never had a plexiglass on the left side of the sink. An interview on 07/17/2024 at 1415 revealed the cart, containing clean supplies, should not be located next to the handwashing sink and eyewash station. The interview revealed that the facility will order a piece of plexiglass to be placed on the side of the sink.”

    Rule V0117. Standard-level.
