What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Bereavement Coordinator E stated, “...The family would not be happy. The IDG would not know if there was follow up with the family…” The surveyor inquired as to whose responsibility it was to develop and follow a bereavement plan of care. Bereavement Coordinator E stated, “...It’s my responsibility...”An interview was conducted with the agency’s Administrator on 8/19/2025 at approximately 12:06 p.m. The surveyor inquired as to how not having a bereavement plan of care for Patient #1 can affect the bereaved family members. The Administrator stated, “...It says that the bereavement plan of care needs to be done...” The surveyor inquired as to whose responsibility it was to develop and follow a bereavement plan of care for the agency’s patients. The Administrator stated, “...The Bereavement Coordinator...”Review of the agency’s policy titled, “ BEREAVEMENT PC.26 Page 1 of 2 Revised 020113” read, in part, “...POLICY...Following the death, the Agency will develop and implement the bereavement plan of care based on the post death risk assessments...Agency will provide a bereavement program for a minimum of 12 months following the death of the patient, to assist grieving families/significant others by introducing coping strategies, caring interventions and community resources...””
