What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Prevention (42 CFR 418.60(a))

    The inspectors’ words, 186 words

“It is required that the hospice follows accepted standards of practice to prevent the transmission of infections and communicable disease, including the use of standard precautions, such as: hand hygiene. This standard was not met as evidenced by document review, interview, and observation. Agency policy and procedure #IPC.11_A2 “Bag Technique and Standard Precautions” (review date 10/30/2024) stated in part, “Perform hand hygiene before and after entering the bag.” In 1 of 6 (17%) home visits observed, the agency did not ensure that hand hygiene was performed when indicated per the agency’s policy. Home Visit 2: The patient elected hospice care on 10/14/2024 with a diagnosis of unspecified severe calorie malnutrition. During the home visit with the Licensed Practical Nurse (LPN) [Personnel Record 2] on 04/23/2025, the LPN reached into the nursing bag to retrieve a protective barrier. The LPN then washed their hands and donned gloves. Hand hygiene was not performed before entering the bag per the agency’s policy. During an interview on 04/24/2025, the Area Executive of Clinical Operations confirmed that the Licensed Practical Nurse did not follow the agency’s policy related to hand hygiene.”

    Rule L0579. Standard-level.

  • Supervision of hospice aides (42 CFR 418.76(h)(1)(i))

    The inspectors’ words, 264 words

“It is required that hospice aides are supervised by a registered nurse who makes an on-site visit to the patient’s home no less frequently than every 14 days to assess the quality of care and services provided by the hospice aide and to ensure that services ordered by the hospice Interdisciplinary Group (IDG) meet the patient’s need. This standard was not met as evidenced by clinical record review, document review, and interview. Agency policy and procedure #C_12A “Supervision of Hospice Aides and LP/LV Colleagues” (review date 12/31/2024) read in part, “At every scheduled visit, the RN is responsible to evaluate the care provided by the hospice aide; it is required that this supervision be completed not less than every 14 days.” In 1 of 19 (5%) clinical records reviewed of patients who received Hospice Aide (HA) services, the agency did not ensure that a Registered Nurse (RN) made an on-site visit to the patient’s home no less frequently than every 14 days to assess the quality of care and services provided by the HA. Clinical Record 13: The patient elected hospice care on 12/10/2024 with a diagnosis of atherosclerotic heart disease. The hospice plan of care dated 12/10/2024 through 03/09/2025 ordered HA services two (2) times weekly beginning 12/10/2024. The HA visits were provided beginning the week of 12/10/2024 as ordered. The HA supervisory visits were provided on 12/19/2024 and 01/09/2025, with 21 days outstanding. In a joint record review and interview on 04/24/2025, the Area Executive of Clinical Operations confirmed that, per agency policy, HA supervisory visits were not completed every 14 days.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
