What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Approach to service delivery (42 CFR 418.56(a))
The inspectors’ words, 214 words
“It is required that the hospice designates a registered nurse member of the IDG to provide coordination of care, ensure continuous assessment of each patient’s and family’s needs, and ensure the implementation of the interdisciplinary plan of care. This standard was not met as evidenced by clinical record review and interview. In 1 of 9 (11%) clinical records reviewed, the agency did not ensure implementation of the interdisciplinary plan of care. Clinical Record 8: The patient elected the hospice benefit on 12/13/2024. The plan of care dated 12/13/2024 to 03/12/2025 included an order for Skilled Nurse (SN) services to be provided 1w13 (once a week for 13 weeks). Clinical record review revealed an extra SN visit was documented on 01/03/2025, the second SN visit provided for the week. There was no documentation in the SN noted dated 01/03/2025 that the visit was a PRN (as needed) visit. During an interview and record review on 01/28/2025, the Administrator stated “There were two SN visits made during the week of 12/24/2024. The visit made on 01/03/2024 was not a PRN visit. This is a known EMR (Electronic Medical Record) issue, the start date of the order didn’t capture correctly when the Registered Nurse entered the order. We are working with the vendor to get this corrected.””

Hospice aide assignments and duties (42 CFR 418.76(g)(2)(ii))
The inspectors’ words, 211 words
“It is required that a hospice aide provides services ordered by the Interdisciplinary Group, included in the plan of care, permitted to be performed under state law and regulation, consistent with the hospice aide training. This standard was not met as evidenced by clinical record review and interview. In 1 of 9 (11%) clinical records reviewed with hospice aide services provided, the agency did not ensure the hospice aide provided services as ordered in the plan of care. Clinical Record 8: The patient elected the hospice benefit on 12/13/2024. The plan of care dated 12/13/2024 to 03/12/2025 included an order for the Hospice Aide (HA) services 1w13 (once a week for 13 weeks). Clinical record review revealed the HA provided two visits the week of 12/15/2024. HA visits were documented on 12/16/2024 and 12/18/2024. The clinical record included an order for HA visits to increase to 3w13 (three times a week for 13 weeks) starting on 12/19/2024. The HA documented one visit was provided on 12/20/2024. During an interview and record review on 01/28/2025, the Administrator stated, “This is a known EMR (Electronic Medical Record) issue. The start date of the order didn’t capture correctly when the nurse entered the order. We are working with the vendor to get this corrected.””
