What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“Observed in Tracer Activities. In 2 of 2 volunteer record reviews, the hospice surveyor noted that the organization did not make sure they provided initial education and training in emergency management to all new volunteers that is consistent with their roles and responsibilities in an emergency. For example, in the volunteer program review, HOSVOL#1 and HOSVOL#2, did not have documentation that they had been provided emergency management training. The inspectors’ words, 118 words

“Observed in Record Review. In 4 of 19 patient records reviewed, the hospice surveyor noted that the organization did not make sure goals were specific to each patient’s needs. For example, in HOSRRHV#1, HOSRRHV#2, HOSRRHV#3 and HOSRRHV#4, the goals were not specific to the patient’s care needs. The goals were written the same for every patient. Examples of this include Pain will be 2/10 while on hospice, anxiety will be 2/10 while on hospice services, diet orders will be maintained while on service, medication management will include the patient understands medications while on hospice services and patient will have a bowel movement every 2 days while on hospice services. For example, in HOSRRNHV#4, the wound order was to cleanse the wound with saline or wound cleanser, apply alginate, crushed Flagyl and to cover with a foam dressing every 3 days, however, in the clinical note, there were several variations of how the wound care was provided, including cleansing with saline and applying triad. There was also documentation where the wound was cleansed with saline and only documented applying the crushed Flagyl but not the algenate prior to covering with a foam dressing. This was reviewed and discussed with the hospice director.”

  • Rights of the patient (42 CFR 418.52(c)(1))

    The inspectors’ words, 197 words

“Based on record review and interview, the agency failed to ensure effective pain management and symptom control in 1 of 2 (P-1) records reviewed, resulting in the potential for harmful medication effects for this patient. Findings include: P-1: This 76-year-old patient was admitted to hospice care on 5/19/22 with terminal diagnoses of Alzheimer’s disease and dementia. The patient was admitted to hospice residence for respite care on 5/16/23. During record review on 1/18/24, it was noted that the patient was ordered Ativan 1 mg every 6 hours as needed for anxiety; Haldol 1 mg every 6 hours as needed for agitation and restlessness - administer for anxiety if Ativan is ineffective; and NORCO 10-325, 1 tablet every four hours as needed for moderate pain (scale of 4-6) or severe pain (scale of 7-10). On three occasions (5/17/23 at 2:30 p.m., 5/22/23 at 8:05 a.m., and 5/22/23 at 2:11 p.m.), the LPN (Staff-F) administered Ativan, Haldol, and NORCO together when the order for Haldol states to administer, “....if Ativan is ineffective.” This finding was reviewed and acknowledged by the administrator (Staff-A), quality RN (Staff-D), and director of post-acute services (Staff-G) during an interview on 1/18/24 at 3:30 p.m.”

    Rule L0512. Standard-level. From a complaint.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

From the hospice

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
