What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Supervision of hospice aides (42 CFR 418.76(h)(1)(i))
The inspectors’ words, 342 words
“Based on record review and interview, the agency failed to ensure a registered nurse (RN) made an onsite supervisory visit no less frequently than every 14 days for Patient #2 who was receiving hospice aide (HA) services for 1 (#2) out of 14 sampled patients. Findings: Patient #2. Review of the patient’s electronic clinical record revealed the patient’s start of care date was 09/27/2023. The patient’s diagnosis included Alzheimer’s disease. The benefit period reviewed was 11/15/2025 to 01/13/2026. Review of the patient’s POC (Plan of Care) for benefit period 11/15/2025 to 01/13/2026 revealed an order for SN effective 11/16/2025 2wk8, 1wk1, 4 prn for urgent symptom management needs/change in condition, (Skilled Nurse visits two times a week for eight weeks, one time a week for one week, four visits as needed for urgent symptom management needs/change in condition.) Further review of POC revealed a change in SN visit orders. The order revealed SN effective 12/21/2025 1wk1, 2wk2, 1wk1, 4 prn resume 2wk visits for 12/28, (Skilled Nurse visit one time a week for one week, two visits a week for two weeks, one visit a week for one week, four visits as needed, and resume two visits a week for the week of 12/28. The patient’s POC revealed an order for HA effective 11/16/2025, 3wk8, 1wk1 (Hospice Aide visits three times a week for eight weeks, one visit a week for one week). The patient’s Visit Note Report for the HA (hospice aide) was reviewed, and the aide was performing the visits with the patient as ordered. There was no evidence found from 12/15/2025 to 01/05/2026, (22 days) that the RN made a supervised visit for the hospice aide between this time frame. On 01/06/2026 at 9:35 a.m., an interview was conducted with S1ED (Executive Director) and after a review of the patient’s electronic clinical records, she verified that the RN did not make a supervisory visit for the HA no less frequently than every 14 days. S1ED confirmed that the RN should have made the supervisory visit within the 14 days.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
