- 7of 10 CMS’s Hospice Care Index
Each figure’s source sits beside it, in its section below.
What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Supervision of hospice aides (42 CFR 418.76(h)(1)(i))
The inspectors’ words, 219 words
“It is required that hospice aides are supervised by a registered nurse who makes an on-site visit to the patient’s home no less frequently than every 14 days to: assess the quality of care and services provided by the hospice aide and ensure that services ordered by the hospice Interdisciplinary Group (IDG) meet the patient’s need. The hospice aide does not need to be present. This standard was not met as evidenced by clinical record review and interview. In 1 of 17 (6%) clinical records reviewed of hospice patients who elected hospice aide services it was revealed that the agency did not ensure that hospice aides were supervised by a registered nurse who made an on-site visit to the patient’s home no less frequently than every 14 days. CLINICAL RECORD 1: Review of the documentation for the benefit period dated 01/26/2026 to 03/26/2026, revealed the hospice aide visit frequency was ordered four times per week, and aide visits were completed as ordered. The hospice aide supervisory visits by the registered nurse were documented as completed on 01/29/2026, 02/03/2026, 02/13/2026, 02/18/2026, 02/24/2026, 03/13/2026 (17 days), 03/19/2026, and 03/26/2026. During an interview with the Executive Director (Personnel Record 5) and the Director of Clinical Services (Personnel Record 6) on 04/07/2026 they stated they would be providing education to the clinical staff.”

Level of activity (42 CFR 418.78(e))
The inspectors’ words, 285 words
“It is required that hospice volunteers provide day-to-day administrative or direct patient care services in an amount that, at a minimum, equals five percent (5%) of the total patient care hours of all paid hospice employees and contract staff. This standard was not met as evidenced by document review and interview. The review of the documented volunteer hours did not provide evidence that hospice volunteers provided day-to-day administrative or direct patient care services in an amount that, at a minimum, equaled five percent (5%) of the total patient care hours of all paid hospice employees and contract staff. The volunteer percentage for the last 12 months from April 2025 to March 2026, averaged 0.81%. During an interview with the volunteer coordinator (Personnel Record 8) on 04/10/2026, she stated that she had recently taken over the role and was aware that the hours were not meeting the requirement, but they were working on increasing the volunteer percentage. Since becoming the volunteer coordinator, she has reached out to current volunteers and strengthened recruitment and retention efforts, resulting in greater engagement. The document review of the volunteer hours lacked evidence that hospice volunteers provided day-to-day administrative or direct patient care services in an amount that, at a minimum, equals five percent (5%) of the total patient care hours of all paid hospice employees and contract staff. This was evidenced by the volunteer percentage for the last 12 months as follows: April 2025 1.03%, May 2025 0.56%, June 2025 1.33%, July 2025 0.23%, August 2025 0.42%, September 2025 0.52%, October 2025 0.25%, November 2025 0.42%%, December 2025 2.40%, January 2026 0.67%, February 2026 0.5%, and March 2026 1.41% this is an average of 0.81% for the last 12 months.”

Discharge or transfer of care (42 CFR 418.104(e)(1)(i))
The inspectors’ words, 190 words
“It is required that if the care of a hospice patient is transferred to a Medicare/Medicaid certified facility, the hospice forwards to the receiving facility a copy of the hospice discharge summary described in HCDT 40.I; and the patient’s record, if requested. This standard was not met as evidenced by clinical record review and interview. In 1 of 1 (100%) clinical record review of a patient that was discharged from hospice services due to admission to facility revealed the agency did not provide the facility with the discharge summary. CLINICAL RECORD 19: The clinical record review revealed there was no documented evidence that the patient’s discharged summary was sent to the facility. The documentation dated 03/06/2026, stated, “Admitted inpatient effective 03/06/2026 to a non-contracted facility. Husband/pt aware of discharge from hospice.” During an interview with the Executive Director (Personnel Record 5) and the Director of Clinical Services (Personnel Record 6) on 04/10/2026, they stated they were sure that the discharge summary, as well as the medication list and plan of care, were faxed to the hospital, but that they were unable to locate the fax cover sheet at this time.”
