The short answer

Record last released by CMS on Oct 23, 2026. We couldn’t read Centers for Medicare & Medicaid Services’ list since Oct 27, 2026: qcor.cms.gov now refuses our reader. This record is as of then.

The last 24 months 4 deficiencies in 1 month of the last 36. Last cited Jan 2026. Most serious: Standard-level. Every month

What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Patient outcome measures (42 CFR 418.54(e)(2))

    The inspectors’ words, 157 words

“Observed in Record Review. In 2 of 5 patient records reviewed, the organization failed to maintain a complete and accurate patient record. During record review for HOSRRHV#1 and HOSRRHV#3, the Hospice Nurse Surveyor observed some discrepancies in the patient record. For example in both records, the nurse documentation at SOC did not mention the patient had previously been on hospice and there were no notations in IDG that the patient had previously been on hospice. The EMR generated a hospice recertification at 60 days, which was completed. There was still no mention that the patient was now in the 3rd benefit period. Also noted on both patients, the certification of terminal illness at SOC from current episodes stated I have reviewed the clinical information prior to recertification and pt is still considered terminally ill but both patients were being brought back onto hospice after revoking several years earlier. This was discussed and reviewed with the Admin/ DPCS.”

    Rule L0535. Standard-level.

  • Review of the plan of care (42 CFR 418.56(d))

    The inspectors’ words, 207 words

“Observed in Record Review. In 3 of 3 patient records reviewed, the organization failed to plan the patient’s care. During record review for HOSRRHV#1, HOSRRHV#2, and HOSRRHV#3, the Hospice Nurse Surveyor observed the patients plan of care and documentation did not contain progress towards goals. For example in HOSRRHV#1, the patient had a goal of In 2 weeks and while in hospice care, patient will have no unusual bleeding or bruising from anticoagulant therapy. This goal was placed at SOC 4 months prior to survey and there was no documentation of progression of the goal. In HOSRRHV#2, the patient had a goal of in 2 weeks and while on hospice care, the patient/PCG/ Family will be able to cope with the disease/ dying process in anticipation of grief. This goal was placed at SOC 4 months prior to survey and not updated and there was no progress towards the goal. In HOSRRHV#3, the patient had a goal of in 2 weeks and while on hospice care, patient will have skin comfort with no exacerbation as evidenced by relieved dry skin r/t eczema. This was placed on all previous episodes and there was no progress towards this goal documented. This was discussed and reviewed with the Administrator/ DPCS.”

    Rule L0553. Standard-level.

  • Level of activity (42 CFR 418.78(e))

“Observed in Tracer Activities. During the Volunteer Program session with the Volunteer Coordinator, the organization did not show evidence its volunteer program had provided day-to-day administrative or direct patient care in an amount that equaled 5% of the total patient care hours of all paid hospice employees and contract staff. The organization had an overall percentage of 2.9%. This was discussed and verified with the Administrator/ DPCS.”

    Rule L0647. Standard-level.

  • Recertification of the terminal illness (42 CFR 418.102(c))

“Observed in Record Review. In 1 of 5 patient records reviewed, the organization failed to reassess its patient’s. During record review for HOSRRHV#3, the Hospice Nurse Surveyor observed the patient began a new benefit period the same day as survey began but he did not have a certification of terminal illness in place for that benefit period. This was discussed and reviewed with the Administrator/ DPCS.”

    Rule L0668. Standard-level.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
