- 10of 10 CMS’s Hospice Care Index
Each figure’s source sits beside it, in its section below.
What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“Observed in Record Review. In 4 of 14 patient records reviewed, it was noted that the organization did not ensure the plan of care goals included outcome measures for specific care activities. For example, a review of the medical record for RRHV#2, revealed that the patient’s terminal illness diagnosis was Sepsis, unspecified. The plan of care did not include a goal statement to evaluate the response to care, treatment, and services related to the diagnosis Sepsis, unspecified.

Example #2: A review of the medical record for RRNHV#4, revealed that the patient’s terminal illness diagnosis was Unspecified Protein-Calorie Malnutrition. The plan of care did not include a goal statement to evaluate the response to care, treatment, and services related to the diagnosis of Unspecified Protein-Calorie Malnutrition.

Example #3: A review of the medical record for RRNHV#6, revealed the following problem statements: Need for skilled Assessment related to infusion therapy [Patient was receiving TPN via a PICC Line]; Need for Skilled Teaching related to Feeding Tube [Patient had a G-tube for venting]. In 1 of 2 episodes of care, the plan of care did not include specific interventions related to Infusion Therapy, PICC Line care, and G-tube care. In addition, the following goal was noted: Pain currently managed at a level acceptable to patient. The comprehensive pain assessment indicated the patient’s pain goal on a verbal descriptor scale was 3. The documented goal was not specific and individualized based on the patient’s assessment.

Example #4: A review of the initial comprehensive assessment for RRNHV#9, revealed the following information: ...history of falls and syncope, multiple rib fractures due to fall, left hemothorax, was hospitalized for severe hydration/hypernatremia, was discharged yesterday. Was ambulating with walker before fall 3 months ago …. patient is now dependent on all 6/6 ADLS. Patient scored at risk for falls on the organization’s fall risk assessment.
