What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of plan of care (42 CFR 418.56(c))
The inspectors’ words, 187 words
“It is required that each patient’s individualized written plan of care identify planned interventions based on problems identified in the initial and updated comprehensive assessments. This standard was not met as evidenced by clinical record review and interview. In 1 of 5 (20%) clinical records reviewed, the agency did not ensure that the patient’s individualized plan of care included planned interventions based on problems identified in the initial and updated comprehensive assessments. CLINICAL RECORD 3: The hospice plan of care dated 11/21/2025 for the 1st benefit period (10/20/2025 to 12/03/2025) included the following: “Problem: 9 wounds. Goal: Have all wounds heal. I: SN (skilled nurse) Wound care as ordered per facility.” This plan of care did not identify the location or specific type of the wounds, nor the care needed. The initial assessment dated 10/20/2025 documented 12 wounds, not 9. An interview and joint clinical record review were conducted with the Administrator and Regional Team on 06/05/2026, during which time it was discussed “there were clearer wound care orders in the ICC (integumentary command center); however, it was agreed these weren’t clear in the plan of care.””

Hospice plan of care (42 CFR 418.112(d)(1))
The inspectors’ words, 287 words
“It is required that hospice care of the patient/resident is stated in a written plan of care established and maintained in consultation with SNF/NF or ICF/IID representatives. The hospice plan of care identifies the care and services that are needed and specifies which provider is responsible for performing the functions that are agreed upon and included in the plan of care. This standard was not met as evidenced by clinical record review and interview. In 2 of 2 (100%) clinical records reviewed of hospice patients in a SNF (skilled nursing facility)/NF (nursing facility), the agency did not ensure a written plan of care specified which provider was responsible for performing functions that are agreed upon and included in the plan of care. CLINICAL RECORD 3: A clinical record review conducted on 06/05/2026 revealed the patient was admitted to hospice three separate times in a skilled nursing facility (SNF) from 02/13/2026 to 02/26/2026, from 12/22/2025 to 01/28/2026, and from 10/20/2025 to 12/03/2025. The clinical record did not include documented evidence of a coordinated care plan outlining the delineation of duties. CLINICAL RECORD 5: A clinical record review conducted on 06/05/2026 revealed that the patient was admitted to hospice care at home on 02/13/2026. After a stay in the hospice-owned inpatient unit for GIP, then respite, the patient was transferred to live in a skilled nursing facility, where she remained until her death on 03/04/2026. The clinical record did not include documented evidence of a coordinated care plan outlining the delineation of duties. An interview and joint record review were conducted with the Administrator and Regional Team on 06/05/2026, and it was confirmed that the clinical record(s) did not contain the facility coordination-of-care forms outlining the delineation of duties.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
