- 9of 10 CMS’s Hospice Care Index

What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Coordination of services (42 CFR 418.56(e)(5))
The inspectors’ words, 477 words
“Based on observation, agency policies and procedures, and interview, it was determined the hospice agency failed to ensure coordination of services with the skilled nursing facility (SNF). Policy Number: 7.9 Revised Date: April 2016 Policy: Hospice provides care to patients who are residents of a SNF/NF... Purpose: To define the roles of Hospice and the facility for patient care. Agency Policy: Coordination of Patient Care Policy Number: 9.13 Revised Date: March 2016 Policy: Hospice staff members regularly communicate to ensure that their efforts are coordinated... Procedure: Hospice has developed and maintains a system of communication and integration to: ... - Ensure that care and services are provided in accordance with the POC... Provide for and ensure the ongoing sharing of information between all disciplines providing care and services in all settings... HV # 3 was admitted to hospice services on 2/16/24 with diagnoses including Cerebral Infarction, Unspecified and Urinary Tract Infection, Site Not Specified. A HV was conducted at the SNF where HV # 3 resided on 3/27/24 at 1:15 PM to observe care provided by Employee Identifier (EI ) # 4, Chaplain. A review of HV # 3’s hospice binder, located at the SNF nursing station, revealed there was no documentation of HV # 3’s POC, IDG meetings, or physician orders within the binder. This was confirmed by EI # 4 at 1:35 PM during the visit. The hospice agency failed to follow policy and provide documentation for the SNF clinical record including the POC and physician orders to ensure coordination of patient care. An interview with a licensed practical nurse at the SNF on 3/28/24 at 4:59 PM via phone confirmed that all hospice documentation brought to the SNF was placed within the patient’s hospice binder maintained at the nurses station.”

Laboratory services (42 CFR 418.116(b)(1))
The inspectors’ words, 303 words
“Based on medical record (MR) review, Roche Diagnostics Package Insert for CoaguCheck Meter and interview, it was determined the hospice agency failed to obtain a Clinical Laboratory Improvement Amendments (CLIA) waiver to allow agency staff to perform CLIA waived laboratory (lab) tests. This deficient practice affected one of 19 MR’s reviewed, including MR # 10, and had the potential to affect all patients served by the agency requiring CLIA waived diagnostic lab tests. Findings include: Package Insert Roche CoaguCheck XS Meter: This insert is for healthcare professional use...This is a CLIA Waived test system. A Certificate of CLIA Waiver (or higher) is required to perform the test. Facilities performing testing must have a CLIA Certificate of Waiver... 1. MR # 10 was admitted to hospice services on 2/5/24 with diagnoses including Dilated Cardiomyopathy and Essential Primary Hypertension. Review of the 2/5/24 Hospice Plan of Care revealed a medication order for Warfarin (Coumadin) 4 milligrams daily, and an order for the Skilled Nurse (SN) to obtain a PT/INR (prothrombin time/international normalized ratio) lab test via bedside monitor weekly. Review of SN Visit Notes dated 2/12/24, 2/20/24, 2/26/24, 3/4/24, 3/11/24, and 3/18/24 revealed the SN obtained a PT/INR by fingerstick blood sample obtaining results utilizing a coagulation meter. Further review of SN visit dated 2/26/24 revealed the PT/INR result was not within normal limits, the SN notified the Nurse Practitioner of the PT/INR result and received a verbal order to adjust the coumadin dose. The hospice agency was performing the CLIA waived PT/INR lab and treating MR # 10 based on the lab result, without having a CLIA waiver. An interview was conducted on 3/28/24 at 6:01 PM with Employee Identifier # 5, Branch Manger, who confirmed the hospice agency did not have a CLIA waiver and the agency owned the Roche CoaguCheck XS Meter.”
