What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Plan Of Care
The inspectors’ words, 389 words
“Plan of care CFR(s): 484.60(a)(1) Each patient must receive the home health services that are written in an individualized plan of care that identifies patient-specific measurable outcomes and goals, and which is established, periodically reviewed, and signed by a doctor of medicine, osteopathy, or podiatry acting within the scope of his or her state license, certification, or registration. If a physician or allowed practitioner refers a patient under a plan of care that cannot be completed until after an evaluation visit, the physician or allowed practitioner is consulted to approve additions or modifications to the original plan. This Standard is not met as evidenced by: Based on record review and interview, the agency failed to ensure the patient’s written individualized POC (Plan of Care) was followed as evidenced by the SN (Skilled Nurse) failing to perform and record blood glucose readings for patient (#8) for 1 out of 17 sampled patients. Patient #8. Review of the patient’s electronic clinical record revealed the start of care date was 10/07/2024. The patient’s diagnoses included Coronary Artery Bypass Graft, Hypertension, and Type 2 Diabetes. The certification period reviewed was 10/07/2024 to 12/05/2024. Review of the patient’s POC for certification period 10/07/2024 to 12/05/2024 revealed an order for skilled nurse 3wk1, 1wk8 (skilled nurse visit three times a week for one week, skill nurse visit one time a week for eight weeks). Further review of the patient’s POC revealed an order for “…Skilled nurse to instruct patient on blood glucose testing. Frequency for blood glucose testing QSNV (Every Skilled Nurse Visit) patient’s therapeutic CBG (Capillary Blood Glucose) range is 70 mg/dl (milligrams per deciliter) to 200 mg/dl…” Review of the patient’s SN visit notes dated 10/07/2024, 10/08/2024, 10/11/2024, 10/17/2024, and 10/28/2024 revealed there was no evidence the SN performed and recorded blood glucose readings. On 10/29/2024 at 1:35 p.m., a telephone interview was conducted with S2RN (Registered Nurse). S2RN confirmed that she did not perform blood glucose testing on the patient for every SN visit. On 10/29/2024 at 2:45 p.m., an interview and review of the patient’s electronic clinical record was conducted with S1ED (Executive Director). S1ED confirmed there was no evidence the SN performed and recorded the patient’s blood glucose readings. S1ED stated the SN should have performed the patient’s blood glucose testing and documented the results in the patient’s clinical record.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
- Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
