What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Discharge or transfer of care (42 CFR 418.104(e)(1)(i))

    The inspectors’ words, 349 words

“It is required that if the care of a hospice patient is transferred to a Medicare/Medicaid certified facility, the hospice forwards to the receiving facility a copy of the hospice discharge summary described in HCDT 40.I; and the patient’s record, if requested. This standard was not met as evidenced by clinical record review and interview. In 2 of 2 (100%) clinical records reviewed of patients who revoked the hospice benefit, revealed the agency did not ensure that the discharge summary was sent to the hospital when the patient transferred to a hospital for aggressive treatment. CLINICAL RECORD 4: The patient elected the hospice benefit on 11/08/2024. On 11/23/2024 the nurse completed the agency document titled “Discharge Summary Live” that was signed by registered nurse (RN), that stated, “discharge to a hospital for aggressive treatment. Patient was admitted to the hospital revoked the hospice benefit.” The “Discharge - Live” document included a summary of the patient’s care while on hospice and included documentation of the patient’s medications, treatments/symptom management and orders, however it was not sent. There was no documented evidence that the discharge summary was sent to the hospital where the patient was transferred for treatment. CLINICAL RECORD 6: The patient elected the hospice benefit on 02/28/2025. On 03/20/2025. the nurse completed the agency document titled “Discharge Summary Live” that was signed by registered nurse (RN), that stated, “discharge to a hospital for aggressive treatment. In an interview and a joint review of clinical records with the Alt Director of Nursing (Personnel Record 2), it was confirmed that the “Discharge summary was completed and there was no documentation in the clinical record that this form was sent to the hospital where the patients were admitted for care.”

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
