What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 531 words

“This is a continuing violation from (event ID: 60B57-H1 Exit Date: 8/9/23). Based on record review and interview, the agency failed to conduct an exercise to test the emergency preparedness and response plan at least once a year to determine if the plan worked and to update any needed changes for 3 of 3 years reviewed. (2023, 2024 and 2025) in that: The agency did not test the EPP at least annually by conducting exercises and failed to maintain documentation of natural or man-made emergencies that took place during 2023, 2024, or 2025. This failure could potentially lead to unidentified issues with the emergency plan and place patients at risk of harm or not having their needs met if the agency was not prepared for an actual emergency event. Findings included: Record review of the agency’s undated Emergency Preparedness Plan binder gave no indication the agency implemented a test to review the emergency preparedness plan in 2023, 2024, or 2025. Record review of the Plan of Correction dated 9/7/23 for survey performed on 8/9/23 indicated the agencies plan for EPP testing was updated and reinforced to address state and federal requirements by conducting an annual exercise of the EPP. Surveyor attempted a telephone interview on 8/19/26 at 11:04 am with the Alternate Administrator/ Alternate Supervising Nurse. No answer. Voicemail message indicated caller was unable to leave a message because the voicemail box has not been set up yet. During an interview and record review on 08/19/26 at 11:43 am., the Administrator/ Supervising Nurse reviewed the EPP binders for the agency and confirmed there was no EPP testing documented for 2023, 2024, or 2025. The Administrator/ Supervising Nurse said she was the Emergency Preparedness coordinator for the agency. The Administrator/ Supervising Nurse said she could not remember if the agency completed testing of the agencies EPP plan. The Administrator/ Supervising Nurse said she knew the EPP plan was implemented with every event such as a hurricane, but it was not documented. The Administrator/ Supervising Nurse said the EPP plan should be tested yearly as needed based on the situation such as a hurricane. The Administrator/ Supervising Nurse said she was responsible for documenting the testing of the EPP system. The Administrator/ Supervising Nurse said the EPP system testing was not documented because the agency did not have active patients. During an interview on 8/19/26 at 12:05 pm, the Administrator/ Supervising Nurse acknowledged EPP testing was on the plan of correction dated 9/7/23. The Administrator/ Supervising Nurse said she did have the EPP testing but she was not sure where it was. Surveyor attempted a telephone interview on 8/19/26 at 12:10 pm with the Alternate Administrator/ Alternate Supervising Nurse. No answer. Voicemail message indicated caller was unable to leave a message because the voicemail box has not been set up yet.Record review of an Emergency Preparedness and Response Plan policy dated 01/01/13 indicated “… As part of the annual internal review, the agency will test the response phase of its Emergency Preparedness and Response Plan in a planned drill if not tested during an actual emergency response. The planned drill may be tabletop exercise, a functional exercise, or some hybrid of both. …””

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
