What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of comprehensive assessment (42 CFR 418.54(c)(6)(i))
The inspectors’ words, 425 words
“It is required that the documented comprehensive assessment include a drug profile that contains the patient’s current: prescription and over-the-counter (OTC) drugs; supplements; herbal remedies; [and] other alternative treatments that could affect drug therapy. The medication review process includes the identification of the following: the effectiveness of drug therapy; drug side effects; actual or potential drug interactions; duplicate drug therapy; [and] drug therapy associated with laboratory monitoring. The assessment includes evidence that common side effects of medication in the hospice populations are anticipated, and, as appropriate, preventive measures are implemented to manage the side effects. The drug profile is updated per the hospice policy and procedure, but, at minimum, at the time of each comprehensive assessment and/or when new medication is added or changes are made to existing medication. This standard was not met as evidenced by clinical record review, interview, and observation. In 2 of 8 (25%) clinical records reviewed and home visits observed, the agency did not ensure that the patient’s drug profile contained the patient’s current prescription drugs. Clinical Record 1/Home Visit 1: The patient elected the hospice benefit on 06/05/2025 with a diagnosis of Alzheimer’s disease. The patient resided in an assisted living facility (ALF). During the home visit with the Hospice Aide (HA) [Personnel Record 6] on 06/17/2025, the hospice medication profile (printed on 06/16/2025) was compared to the ALF medication profile (printed on 06/17/2025). The hospice medication profile documented the following medication not listed on the ALF medication profile: Tramadol 50 mg tablet, take one (1) tablet twice daily, ordered 06/05/2025. Clinical Record 2/Home Visit 2: The patient elected the hospice benefit on 05/07/2025 with a diagnosis of Parkinson’s disease. The patient resided in an ALF. During the home visit with the Registered Nurse/Director of Clinical Services (RN/DCS) [Personnel Record 3] on 06/17/2025, the hospice medication profile (printed on 06/16/2025) was compared to the ALF medication profile (printed on 06/17/2025). The hospice medication profile documented the following medications not listed on the ALF medication profile: acetaminophen 325 mg tablet, take two (2) tablets every 6 hours as needed, ordered 05/07/2025, and Senna 8.6-50 mg tablet, take two (2) tablets twice daily as needed, ordered 05/07/2025. Additionally, the hospice medication profile documented morphine 100 mg/5 ml, take 5 mg every 4 hours as needed. The ALF medication profile documented morphine 100 mg/5 ml, take 5 mg every 3 hours as needed. During interviews, the Director of Clinical Operations, who attended all home visits on 06/17/2025, confirmed the discrepancies, stating that it was the process “to check the meds every visit.””
