What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of plan of care (42 CFR 418.56(c))
The inspectors’ words, 97 words
“Observed in Record Review. In 10 of 10 patient records reviewed, for active patients, the nurse surveyor noted that the goals were not consistently measurable. For example, “Pain will remain in comfortable level during care” and Facilitate Bowel elimination and comfort. There were no short-term goals at start of care to address issues of immediate needs such as understanding medication management, symptom management, pain management, or safety, most time frames for completion were at end of certification period and target dates are not being consistently updated. Finding was confirmed by the Office Manager and the Administrator/DPCS Designee.”
-
Supervision of hospice aides (42 CFR 418.76(h)(1)(i))
“Observed in Surveyor review but corrected onsite pending acceptable Evidence of Standards Compliance. In 2 of 7 patient records reviewed, with a Hospice Aide it was noted by the nurse surveyor that Hospice Aide Supervisory visits were not documented the required every fourteen days. For RRNHV#2 the HHA Supervisory visit was 7 days late and for RRNHV#6, the HHA Supervisory visit was 8 days late. Findings were verified with the Office Manager and the Administrator/DPCS Designee.”
-
Recruiting and retaining (42 CFR 418.78(c))
“Observed in Tracer Activities. It was noted by the nurse surveyor that the Volunteer Coordinator did not document retention activities that he did with/for his volunteers. Finding was discussed with the Administrator/DPCS Designee and Office Manager.”
-
Retrieval of clinical records (42 CFR 418.104(f))
The inspectors’ words, 105 words
“Observed in Record Review. In 10 of 10 patient records reviewed, it was noted by the nurse surveyor that entries into the electronic medical record by the skilled nurse and social worker for RRHV#1, RRNHV#2, and RRNHV#4 were not made within the organizations’ defined time frame of 48 hours (Submission of Notes, Policy No 2-005 1A). Additionally, numerous notes in all of the records reviewed did not have the final sign-off by Quality Assurance (QA) which completes the note and affixes the final electronic signature. Some were more than a month without QA review. Finding was verified by the Office Manager and the Adminstrator/DPCS Designee.”
-
Federal, state, local laws & regulations (42 CFR 418.116)
The inspectors’ words, 133 words
“Observed in Record Review. In 1 of 1 patient records reviewed, for live discharge due to extended prognosis, it was noted by the nurse surveyor that the Hospice did not have a process in place to ensure that a Notice of Medicare Non-Coverage (NOMNC) was signed by the patient or patient’s representative at least two days prior to a planned discharge as required. For RRDC#1, the patient was discharged for extended prognosis, NOMNC was signed by the patient’s daughter on the day of discharge even though discharge planning had begun a week prior and was documented in the EMR. Additionally, the NOMNC form did not include the phone number or name of the agency’s Quality Improvement Organization (QIO). Finding was discussed with the Administrator/Director of Patient Care Services, Designee and the Office Manager.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for hospices, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
