The short answer

Record last released by CMS on Oct 23, 2026. We couldn’t read Centers for Medicare & Medicaid Services’ list since Oct 27, 2026: qcor.cms.gov now refuses our reader. This record is as of then.

The last 24 months 6 deficiencies in 1 month of the last 36. Last cited May 2025. Most serious: Standard-level. Every month

What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

May 8, 2025: reaccreditation inspection by The Joint Commission6 deficiencies
  • Initial & comprehensive assessment of patient (42 CFR 418.54)

“Observed in Patient Home. In 1 of 14 home visits, surveyor noted that the organization failed to conduct a patient specific reassessment of the patient per the organization’s policy. For example, the nurse did not perform a complete head-to-toe skin assessment of the patient who has a high risk for skin breakdown. This was reviewed with the Administrator.”

    Rule L0520. Standard-level.

  • Content of plan of care (42 CFR 418.56(c)(1))

    The inspectors’ words, 116 words

“Observed in Record Review. In 1 of 14 records reviewed, surveyor noted that the organization failed to develop a plan of care that includes all services needed for the palliation and management of the terminal illness and related conditions. For example in RRNHV#4, surveyor noted that the nurse documented that patient had a left toe fungal rash/erythema, however, there was no care plan for impaired skin integrity. The nurse documented that that patient had abnormal breath sounds, however there was no care plan for impaired lung function. The patient’s medication profile indicated that patient has scheduled and PRN Tylenol for pain, however, there is no care plan for pain. This was reviewed with the Executive Director.”

    Rule L0546. Standard-level.

  • Coordination of services (42 CFR 418.56(e)(2))

    The inspectors’ words, 93 words

“Observed in Record Review. In 1 of 4 home visits, surveyor noted that the organization failed to provide care, treatment, or services in accordance with orders or prescriptions, as required by law and regulation. For example, during subsequent record review HV#1, surveyor noted that the nurse did not provide wound care according to the physician’s order which stated clean 2nd left toe diabetic ulcer with wound cleanser and apply iodine (to diabetic ulcer) every three days; the nurse did not cleanse the wound before applying iodine. This was reviewed with the Executive Director.”

    Rule L0555. Standard-level.

  • Prevention (42 CFR 418.60(a))

“Observed in Patient Home. In 1 of 3 visits, surveyor observed that the organization failed to implement activities for the prevention and control of infections and communicable diseases per their policy. For example, in HV#1, the nurse did not disinfect her hands after providing patient care that included assessment of vital signs and wound care, increasing the potential for spreading infectious/communicable diseases. This was reviewed with the Executive Director and Director of Patient Care Services.”

    Rule L0579. Standard-level.

  • Cost saving (42 CFR 418.78(d))

“Observed in Tracer Activities. During the Volunteer Program Review, surveyor noted that the organization failed to document/achieve cost savings that the hospice would have incurred if paid employees occupied positions engaged in administrative or direct patient care roles. For example, in year 2023, the organization did not have active volunteers, subsequently, the organization was not able to demonstrate cost savings through the use of volunteers in administrative/direct patient care roles. This was reviewed with the Executive Director.”

    Rule L0646. Standard-level.

  • Level of activity (42 CFR 418.78(e))

“Observed in Tracer Activities. During the Volunteer Program Review, surveyor noted that the organization failed to provide at least 5% of the total patient care hours of all paid hospice employees and contract staff. For example, for year 2023 the total day to-day administrative or direct patient care was 0% of all paid hospice employees and contract staff. This was reviewed with the Executive Director and Bereavement Coordinator.”

    Rule L0647. Standard-level.

What do families say?

  • CMS gives no family caregiver survey rating: “The number of cases is too small to report Star Ratings.”
  • In CMS’s survey of family caregivers (Oct 1, 2023 to Sep 30, 2025): 81% rated it 9 or 10 out of 10, and 87% would definitely recommend it.
  • CMS’s Hospice Care Index: 10 of 10 (Jan 1, 2023 to Dec 31, 2024).
  • 100% of patients were assessed on all seven of CMS’s measures at the start of care (Oct 1, 2024 to Sep 30, 2025); 55.1% had visits from a nurse or social worker on at least 2 of their last 3 days (Jan 1, 2023 to Dec 31, 2024).

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

From the hospice

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
