What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Rn Performs Assessment
The inspectors’ words, 359 words
“RN performs assessment CFR(s): 484.55(a)(1) A registered nurse must conduct an initial assessment visit to determine the immediate care and support needs of the patient; and, for Medicare patients, to determine eligibility for the Medicare home health benefit, including homebound status. The initial assessment visit must be held either within 48 hours of referral, or within 48 hours of the patient’s return home, or on the physician or allowed practitioner - ordered start of care date. This Element is not met as evidenced by: Based on interview and record review, the agency failed to conduct an initial assessment within 48 hours of the completed referral for 4 of 17 patients (#504, #505, #509, and #511). Failure to conduct the assessment within 48 hours could result in a delay of necessary medical treatment. Findings Included: 1. The surveyor reviewed patient records for four patients. The records showed: a. Patient #504 had a completed referral date of 02/14/24. The patient had a start of care date of 02/20/24. There was no documentation evident to determine the reason for the 4-day delay in the start of care. b. Patient #505 had a completed referral date of 03/15/24. The patient had a start of care date of 03/18/24. There was no documentation evident to determine the reason for the 1-day delay in the start of care. c. Patient #509 had a completed referral date of 12/29/23. The patient had a start of care date of 01/02/24. There was no documentation evident to determine the reason for the 2-day delay in the start of care. d. Patient #511 had a completed referral date of 03/22/24. The patient had a start of care date of 03/26/24. There was no documentation evident to determine the reason for the 4-day delay in the start of care. 2. On 04/03/24 at 1:50 PM and 04/05/24 at 2:50 PM, the Surveyor interviewed the agency’s Clinical Services Director (Staff #502) about the delay in the start of care for the above patients. Staff #502 confirmed the accuracy of the completed referral dates and stated that there was no justification documented to indicate the reason for the delayed start of care.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
- Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
