The short answer

Record last released by CMS on Oct 21, 2026.

The last 24 months 1 deficiency in 1 month of the last 36. Last cited Jul 2024. Most serious: Element-level. Every month

Each figure’s source sits beside it, in its section below.

What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Reviewed, Revised By Physician Every 60 Days

    The inspectors’ words, 226 words

“Reviewed, revised by physician every 60 days CFR(s): 484.60(c)(1) The individualized plan of care must be reviewed and revised by the physician or allowed practitioner who is responsible for the home health plan of care and the HHA as frequently as the patient’s condition or needs require, but no less frequently than once every 60 days, beginning with the start of care date. This Element is not met as evidenced by: Based on record review and interview, the agency failed to have the Plan of Care (PoC) reviewed and approved by the physician no less frequently than every 60 days, beginning with the SOC (Start of Care) date. This failed practice was identified for 1 (#4) of 7 (#1-#7) sampled patients whose clinical records were reviewed. Findings: Review of Patient #4’s clinical record revealed a SOC date of 04/03/2024. Review of the PoC/485 for the certification period 04/03/2024 to 06/01/2024 revealed the physician’s review and approval signature was dated 06/05/2024, which was 63 days after the SOC date. An interview with S1ADM was conducted on 07/10/2024 at 11:35 a.m., and the PoC for Patient #4 was reviewed. S1ADM stated the patient’s physician should review and approve all patient’s PoC/485’s within 60 days of the SOC date. S1ADM confirmed Patient #4’s aforementioned PoC/485 was not reviewed and approved within the required 60-day period and should have been.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
  • Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
