What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Poc Completed/Signed By Idt & Patient

    The inspectors’ words, 514 words

“POC-COMPLETED/SIGNED BY IDT & PT CFR(s): 494.90(b)(1) The patient’s plan of care must- (i) Be completed by the interdisciplinary team, including the patient if the patient desires; and (ii) Be signed by the team members, including the patient or the patient’s designee; or, if the patient chooses not to sign the plan of care, this choice must be documented on the plan of care, along with the reason the signature was not provided. This Standard is not met as evidenced by: 32734 Based on medical record review and staff interview, it was determined that the facility failed to ensure that two of two home hemodialysis (HHD) patients (P#1 and P#2) sampled, had a Patient Care Plan that reflected participation of all members of the Interdisciplinary Team (IDT), including the patient or patient representative. This lack of IDT collaboration and participation had the potential failure to identify new patient concerns and outcomes that were not achieved. This deficient practice had the potential to negatively affect the health and safety of 13 HHD patients who were under the care of this facility. Findings include: During a review of medical records for P#1 and P#2, the following was revealed: - Review of plan of care for P#1 dated 6/19/24; and P#2’s plan of care dated 11/30/23, showed a meeting was held and signed by the IDT but there was no signature on the plans of care by neither patient that he/she reviewed the patient care plan or attended the meeting. - Review of Policy: 45283 titled, “Comprehensive Interdisciplinary Assessment and Plan of Care” version 6 dated 7/3/23 showed the following: The Comprehensive Interdisciplinary Assessment and Plan of Care must be developed and implemented by an Interdisciplinary Team (IDT) at a minimum, the patient or patient’s disignee, a registered nurse, attending physcian (or physician extender) where allowed by state regualtions), qualified master’s degree level social worker and qualified registered dietitian. ... The Plan of Care should be signed at the time of the interdisciplinary team meeting for those attending in person. If unable to sign at the time of the meeting or if attending remotely the care plan must be signed by the IDT member within 30 days of the IDT meeting. - Review of facility’s policy (no number assigned) titled, “Patient Rights and Responsibilities” version 2 dated 4/4/12 showed the following: ...The patient has a right to be informed and participate if desired, in all aspects of his or her care. During an interview with the Home Therapies RN (Registered Nurse) on 7/3/24 at 11:40 a.m., she was asked about the process for reviewing the plan of care with the patient. She stated “we review the plan of care when the patient comes in to meet with the nurse for monthly labs and assessment. If we miss getting it signed at that time, we will review the plan of care during monthly rounds with the doctor”. However, there was no documentary evidence that the plan of care was reviewed with P#1. A telephone call was made to P#1 but P#1 did not answer or return the call.”
