What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Ic - Wear Gloves/Hand Hygiene
The inspectors’ words, 391 words
“IC-WEAR GLOVES/HAND HYGIENE CFR(s): 494.30(a)(1) Wear disposable gloves when caring for the patient or touching the patient’s equipment at the dialysis station. Staff must remove gloves and wash hands between each patient or station. This Standard is not met as evidenced by: 32734 Based on observation, staff interview, a review of Centers for Disease Control and Prevention (CDC) recommendations, and a review of the facility’s Policy and Procedures (P & P), it was determined that the Infection Control Committee failed to implement facility P & P relative to infection control and prevention, as evidenced by failure of one of one Patient Care Technician (PCT AA) observed, to adhere to infection control techniques to prevent cross contamination. This failure had the potential to expose patients to bloodborne pathogens in the dialysis environment. The facility census was 38. Findings include: During observation in the Patient Treatment Room on 3/12/25 at 10:15 a.m., PCT AA set the blood flow rate, adjusted the dialyzer and keyboard (dirty tasks), then purged saline from the venous bloodline and proceeded to initiate treatment (clean task) on a patient (P) (P#6) at Station (S) 11. He then proceeded to S10 at 10:20 a.m., began setting up the machine for P#7 and opened/obtained a sterile dialyzer and placed it in the holder while he wore the same contaminated gloves. PCT AA did not change gloves or perform hand hygiene when going from dirty task to a clean task and between stations, until he was asked to do so by the Clinical Specialist (CS). During an interview on 3/12/25 at approximately 10:40 a.m., PCT AA stated, “I knew I messed up”. According to CDC RR-5 “Recommended Infection Control Practices for Hemodialysis Units at a Glance”, staff must wear disposable gloves when caring for the patient or touching the patient’s equipment at the dialysis station. Staff must remove gloves and wash hands between each patient or station. - A review of the facility’s Policy #C-IC-0010 titled, “Infection Control and Precautions for All Patients” with latest revision date of July 2023, showed the following: Hand Hygiene: 10. Staff will perform hand hygiene: i. Using soap and water or antibacterial hand sanitizer before/after all patient contact and before donning and after doffing PPE, including gloves. ii. After contact with integrated keyboard or chairside computer, regardless of whether contact occurred through gloved or ungloved hands.”
