The short answer

- May 212026 the latest visit on record
- 155 children, its licensed capacity
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

“Based on staff interviews, a review of classroom video footage, and staff written statements it was determined that on 8/18/2022 Child A (age 3) was left in the Intermediates classroom beyond the sight and sound supervision of staff members for approximately 2 minutes. At approximately 10:56 Staff 1 appears to call the name of Child A, however the child did not proceed to the playground. Instead, Child A went to the sink in the corner of the classroom. At the same time, Staff 1 is seen completing the name-to-face roll call process, closing the classroom door, and proceeding outside with the children. At this time, the video shows Child A walking around the classroom alone and beyond sight and sound supervision of staff members. Roughly 2 minutes later, at approximately 10:58, Child A is viewed opening the playground door and exiting the classroom. Through staff interviews and written statements, it was determined that of the three staff present at the time, none were aware that Child A remained in the classroom beyond sight and sound supervision. The email included pictures showing a scratch with broken skin and a dark mark next to the scratch on the child’s left arm. The email also stated that the child indicated the marks were caused by Staff B. 3 - Upon receiving the email notification, Staff A did not follow the center’s policies requiring them to report the information to Child Protective Services. 4 - The center’s written behavioral guidance policy states that “children learn best when they are comfortable and safe....actions which disrupt or undermine this positive environment are prohibited.” The policy further details examples of prohibited actions, including “any type of abuse of children.” 5 - On 12/30/2021 Staff B did not follow the center’s behavioral guidance policy when they grabbed Child 1 (age 2) by the left arm to move the child away from the sink after telling the child to quit playing with the water. Staff B scratched Child 1 (age 2) on the left arm leaving broken skin and a dark mark next to the scratch. 6 - Staff A and Staff B signed the center’s acknowledgment form that these written policies were received and reviewed as part of their Licensing Orientation.”

“Based on staff interviews, it was determined that behavioral guidance was not constructive in nature, was not age and stage appropriate, and was not intended to redirect children to appropriate behavior. Evidence: On 12/30/2021 Staff B grabbed the left arm of Child 1 (age 2) after the child did not stop playing in the water at the sink. In grabbing the child by the arm, Staff B left a scratch mark that broke the skin and a dark mark next to the scratch.”

The inspectors’ words, 163 words
“Based on staff interviews and photographic evidence, it was determined that one staff person engaged in the forbidden action of roughly handling a child as punishment. Evidence: 1 - On 12/30/2021 Child 1 (age 2) received injuries to the left arm while in care. Pictures of the injuries show a scratch that broke the skin and a dark mark next to the scratch. Both injuries were caused when Staff B grabbed the child by the arm. 2 - During interviews, Staff B acknowledged that these marks were caused when they “grabbed the child by the wrist with one hand” and “probably because they have very long fingernails.” Staff B also acknowledged that this incident took place because the child had made a mess while playing in the water at the sink and did not stop when told to do so. 3 - During interviews with Staff C, it was further determined that Child 1 (age 2) was observed crying after receiving the injuries.”

The inspectors’ words, 96 words
“Based on staff interviews, it was determined the center did not maintain written record of a child’s serious or minor injuries in which entries were made the day of occurrence. Evidence: On 12/30/2021 Child 1 (age 2) was injured while in care at the center. On this day, the child received a scratch on the left arm which broke the skin, and a dark mark next to the scratch caused from being grabbed by the arm. These injuries were reported to Staff A on 12/31/2021 but written record of the injuries was not completed or maintained.”

What the record doesn’t say
- Virginia doesn’t grade how serious a violation is, so no severity is shown.
- Virginia publishes a complaint’s allegations and findings only when its inspection found a violation and the records are linked.
- Virginia doesn’t publish staffing levels or owners for individual providers.
- A plan of correction sent late, or not at all, isn’t online: Virginia says to ask the inspector.
- Virginia’s pages show about the last five years of inspections; for older ones, Virginia says to contact the licensing inspector.
- Virginia says this information may not be accurate, up to date or complete, and shouldn’t be the only source in choosing care.
