What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Aug 20, 2026OCCL Standards Complaint: SubstantiatedOn July 28, 2026, OCCL received a complaint alleging the center failed to notify Parent #1 of Child #1’s swollen eye, failed to provide an injury report, and failed to follow its policy for responding to a parent complaint. An unannounced investigation was conducted on August 4, 2026. Staff #1 confirmed Child #1’s left eye was swollen after nap, ice was applied, Parent #1 was not informed at the time the swelling was noticed nor at pick-up, and an incident/injury report was not completed. This evidence substantiates the allegation concerning lack of parent notification of important information regarding a child and failure to provide an accident/injury report. Regarding the allegation of not following the policy of responding to parents complaints, the investigation showed Administrator #1 spoke with Parent #1 about their concerns and reviewed the requested video footage by pick-up time. The information obtained did not establish that the center failed to follow its policy for responding to Parent #1’s complaint. Additional concerns noted during this investigation include administering medication with an incomplete Medication Administration Record (MAR) and not following medication timing as outlined in the Delaware Administration of Medication Self-Training Guide. Based on the results of the investigation, this complaint was: Substantiated
Jul 7, 2026OCCL Standards Complaint: No Evidence to SubstantiateOn June 5, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of not releasing a child to a person on a pick-up list as noted in the parent handbook. An unannounced visit was attempted on June 15, 2026, and a second visit was conducted on June 24, 2026. Based on documents and as well as staff interviews conducted, there is no evidence to substantiate this complaint for not releasing a child to a person on a pick-up list as noted in the parent handbook requirement. An interview with Administrator #1 and documents reviewed show that center policy was followed and Child #1 was picked up at 1:30 pm. Based on the results of the investigation, this complaint was: No Evidence to Substantiate.
May 29, 2026OCCL Standards Complaint: No Evidence to SubstantiateOn May 12, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of not using positive behavior supports and failing to use respectful communication with parents. An unannounced visit was conducted on May 19, 2026. Based on documents and video footage reviewed as well as staff interviews conducted, there is no evidence to substantiate the complaint. Three staff members were interviewed separately and stated they did not observe Child #1 getting pushed and noted Administrator #1 kept a calm demeanor during this incident. Video footage showed Administrator #1 did not place their hands on Child #1. Findings: Based on the results of the investigation, this complaint was: No Evidence to Substantiate.
Nov 24, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on November 4, 2025, regarding concerns of lack of supervision. An investigation was conducted on November 10, 2025. There is no evidence to substantiate this complaint for lack of supervision based on staff interviews conducted and attendance records reviewed. Staff members interviewed stated Child #2 was not observed inappropriately touching Child #1. Attendance records reviewed reflected classrooms within correct staff-to-child ratios.
Aug 1, 2025OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on July 22, 2025, regarding concerns of improper discipline and inappropriate care of children and not following diapering policies. An investigation was conducted on July 28, 2025. There is no evidence to substantiate this complaint for improper discipline or inappropriate care of children based on staff interviews and video footage reviewed. Administrator #1 and Staff #1 interviewed stated they have never witnessed or used any improper discipline, nor have they witnessed inappropriate care of children. Video footage reviewed showed appropriate interactions between staff and children. There is no evidence to substantiate this complaint for not following diapering policies based on staff interviews. Staff interviewed stated they follow posted diaper changing policies for all diaper changes.
Jun 24, 2024OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on May 2, 2024, regarding failure to follow termination policies and not using respectful communication. An investigation was conducted on May 15, 2024. There is no evidence to substantiate this complaint because all documentation reviewed and statements from the administrator indicate policies were followed and Ms. DeBerry-Hill did not engage in disrespectful tones, words, or behavior.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
