What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Aug 20, 2026OCCL Standards Complaint: No Evidence to Substantiate with ConcernOCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On July 14, 2026, the Office of Child Care Licensing (OCCL) received a complaint alleging concerns of not using positive behavior supports, not using respectful communication with families, and discriminatory behavior, contending on July 13, 2026, Child #1 was yelled at and grabbed by the arm by Staff #1 resulting in injury, and stating Staff #1 was verbally and physically intimidating toward the Reporter. It was alleged Staff #1 applies discipline differently based on a child s race. An investigation was conducted on July 21, 2026. There is no evidence to substantiate this complaint. Staff #1 denied ever placing their hands on Child #1. Multiple staff members and Administrator #1 were interviewed, and all stated they did not observe Staff #1 have physical contact with Child #1 on July 13, 2026. Staff #1, #3, and #4 submitted written statements and investigative notes. According to Staff #4 s notes, video footage reviewed by facility administration did not depict physical contact between Staff #1 and Child #1. No written accident or injury report documenting a mark or bruise to Child #1 s arm was located in the record provided to OCCL, and Staff #1 stated they were not made aware of any such injury. There is no evidence to substantiate this complaint for not using respectful communication with families based on staff interviews, written statements, and Staff #4 s accounts of review of video footage. Staff #1 and all staff interviewed denied Staff #1 raised their voice at Parent #1 or used any intimidating body language. According to Staff #4 s investigative notes, video footage reviewed by facility administration did not show Staff #1 displaying aggressive or intimidating body language toward Parent #1. Staff members present during the interaction described Parent #1, rather than Staff #1, as the individual whose voice was raised. There is no evidenc e to substantiate this complaint for discriminatory behavior toward children based on interviews with staff. Staff #1 denied treating children differently based on race and Administrator #1 and Staff #2 denied observing Staff #1 treat children differently based on race. Behavior incidents for Child #1 were written on July 7 and July 13, 2026, and corroborated by multiple staff accounts. A concern noted during this investigation was Child #1 s Individualized Education Program (IEP) was not up to date, and no recent copy of their IEP was available at the visit. There was no documentation that the facility attempted to receive updated IEP materials from Child #1 s parent or guardian.
Aug 16, 2024OCCL Standards Complaint: No Evidence to Substantiate with ConcernOn May 20, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding improper discipline. OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. An investigation was conducted on July 26, 2024. Based on interviews conducted and video reviewed, there is no evidence to substantiate this complaint for improper discipline; however, there is a concern that the staff did not intervene quickly to deescalate child behaviors to ensure the safety of children.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
