The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| May 20, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On April 20, 2026, the Office of Child Care Licensing received a complaint regarding allegations of a staff member bullying the children, the children hearing the staff member speaking badly about the children and parent and not following the center’s policy for suspension. An investigation was conducted on April 29, 2026. Based on interviews and review of incident reports, there is no evidence to substantiate this complaint. All administrators and staff interviewed stated multiple meetings have been held regarding the behavior of Child #1 and Child #2, and a behavior plan was put into place for Child #1, but Parent #1 refused to sign it. Administrators interviewed stated Parent #1 instructed Child #1 and Child #2 not to listen to staff, and the facility followed their suspension policies regarding this incident. All administration and staff interviewed denied speaking badly about any children or families in front of children. |
| Sep 30, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On September 10, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding failing to use appropriate positive behavior supports. An investigation was conducted on September 17, 2025. Based on interviews conducted and video footage reviewed, there is no evidence to substantiate this complaint for failing to use appropriate positive behavior supports. |
| Apr 24, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | On April 2, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding pest infestation, not following policy for informing a parent of an incident or concern regarding the child, and not following requirements for sleep equipment. An unannounced visit was conducted on April 10, 2024. There is no evidence to substantiate this complaint regarding pest infestation. Upon speaking with the administrator, the child care center has routine daily, weekly, and monthly cleaning which is documented by receipts from the companies. LS Strachan observed there were no pests present. There is no evidence to substantiate not following requirements for sleep equipment. Upon interviewing the administrator and inspecting the sleep equipment, the child care center is following the requirements for OCCL. This investigation is not substantiated for not following policy to inform a parent of an incident or concern regarding the child. Upon interviewing the administrator, a copy of the staff communication to the family was presented which documented the concern by staff members. |
| Feb 23, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing received a complaint on February 5, 2024, regarding staff not following correct diapering procedures. An investigation was conducted on February 15, 2024. Based on the results of this investigation, there is no evidence to substantiate that staff are not following correct diapering procedures because both staff who were interviewed stated they follow proper diapering procedures in the classroom. Also, daily logs from the one-year-old classroom show children s diapers are being checked hourly, diaper cream is applied if necessary or requested, and diapers are being changed as required. |
| Dec 6, 2023 | OCCL Standards Complaint: Substantiated | On November 17, 2023, a complaint was called into the Child Abuse and Neglect Report Line and not accepted for investigation by the Institutional Abuse Unit. It was sent to the Office of Child Care Licensing on November 20, 2023, and accepted for investigation for improper discipline. An investigation began on November 20, 2023, and ended on November 30, 2023. The complaint is substantiated for improper discipline based on video footage viewed and interviews with staff members. Video footage showed staff grabbing and pulling Child #1 s arm, yelling at Child #1, sitting them down forcefully on the ground, and pulling Child #1 s jacket hood while trying to redirect Child #1. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
