The short answer

What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Nov 2, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On October 12, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns involving accident and injury reporting, communication of developmental and behavioral concerns, referral requirements for outside services, and the program’s application of written policies. An investigation was conducted on October 21, 2026. Based on the investigation, there is no evidence to substantiate the allegation involving failure to document or report injuries sustained by Child #1. Licensing Specialist (LS) #1 reviewed five completed accident and injury reports involving Child #1. LS #1 also obtained nine weeks of daily health screening documentation records that included Child #1 within the Preschool D classroom and reviewed an example of electronic communication between Staff #1 and Grandparent #1 regarding an observed injury. Administrator #1 reported staff are expected to complete injury reports when injuries occur, including situations involving an unknown cause. There is also no evidence to substantiate the allegation involving inappropriate communication of Child #1’s developmental or behavioral concerns. Administrator #1 and Staff #1 reported concerns involving Child #1’s speech and social-emotional development were identified through classroom observations, developmental screening, and information provided by Grandparent #1 during the August 24, 2026, home visit. The ASQ screening results were communicated to Parent #1, and program administration engaged in discussions regarding Child #1’s developmental needs and potential additional services. The program also provided written communication outlining the expectations associated with developmental referrals and classroom attendance. LS #1 reviewed the program’s referral policy and confirmed the parent handbook includes provisions requiring family participation in recommended developmental evaluations. The policy also identifies potential program suspension or expulsion when families r efuse or consistently fail to cooperate with evaluation and registration procedures. Although the policy identifies potential consequences for nonparticipation, the investigation did not establish Child #1 was suspended or terminated solely because the family had not obtained outside services. Administrator #1 reported the family was informed of the program’s referral requirements and encouraged to participate in the evaluation process. Regarding Child #1’s arrival times, Administrator #1 reported Grandparent #1 was informed of the program’s requirement for children to arrive by 9:00 a.m. LS #1 reviewed the written arrival policy and confirmed the policy establishes a 9:00 a.m. arrival deadline and procedures for excused late arrivals. Information obtained during the investigation supports the program’s communication regarding arrival expectations being consistent with the written parent handbook. |
| Nov 24, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On November 10, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding a concern of improper discipline. An investigation was conducted on November 13, 2025. At the conclusion of the investigation, there is no evidence to substantiate this complaint based on the interviews with Administrator #1 and Staff #2. Administrator #1 conducted an internal investigation, the findings showed no evidence of improper discipline because Staff #1 denied pinching Child #1 and Child #1 s cot was not visible on the video footage. Staff #2, who was also in the classroom, denied witnessing any improper discipline and stated Child #1 did not show any signs of being hurt or uncomfortable on November 6, 2025. Video footage was reviewed by LS #1 and as Administrator #1 stated, Child #1 s cot was unable to be viewed. Administrator #1 noted there is a possibility of Staff #1 badge causing a bruise on Child #1 s ribcage if Child #1 requires being held to fall asleep for nap. |
| Oct 28, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On October 6, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding concerns of not following the parent handbook policies for termination of care. An investigation was conducted on October 20, 2025. Based on the findings of the investigation, there is no evidence to substantiate the complaint. Facility Director (FD) #1 and Administrator #1 made decisions based on communications between Staff #1, FD #1, and the family service worker. Staff #1 initially requested disenrollment for Child #1 on September 18, 2025, then later rescinded the request through the family service worker. Staff #1 also requested two additional weeks of enrollment, which was approved through October 3, 2025. Attendance rosters provided by FD #1 confirmed this arrangement. FD #1 re-enrolled Child #1 on October 7, 2025, after two other children withdrew due to relocation. Child #1 returned to the program on October 8, 2025, and remained enrolled at the time of the investigation. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
