The product, in the report: Anchor Hocking Glass 9 by 13, Baking Dishes 3 Quart
I am writing to formally report a serious product safety incident involving an Anchor Hocking 9 by 13, 3-quart glass baking dish that shattered while being used in my oven. The baking dishes were purchased through Walmart on August 9, 2026. I purchased two Anchor Hocking Glass 9 x 13 Baking Dishes, 3 Quart, for a total of $12.36. During normal cooking use, one of the dishes suddenly shattered inside the oven. The glass broke into numerous pieces and scattered throughout the oven, including across the oven racks, heating area, bottom of the oven, and into and around the food being prepared. Fortunately, I have preserved photographic evidence of the incident. I also have photographs showing the Anchor Hocking name molded into the remaining glass as well as my Walmart purchase documentation confirming the date and product purchased. This incident is particularly concerning because the product was purchased only recently and was being used as a baking dish for its intended purpose.

The product, in the report: 8”x8” Anchor Hocking Glass Cake Pan - Clear. It is not measured at 8”x8” but is labeled as such.
I had purchased an 8”x8” glass baking dish from Walmart under the brand Anchor Hocking Glass a few weeks ago. I did not use it until a week ago, and used it at 375 degrees. It is directed to use under 400 degrees. It was fine in the oven that day and for a few more in the fridge. I had made green bell pepper casserole. It was sitting on my counter for about two days at room temperature, with nothing on top of it, waiting to be washed. I left for work last night and everything was normal, but I came home this morning and stepped on glass in my kitchen floor. I began searching for the source and found the baking dish had shattered while sitting on my counter. I am unsure if my dog has any glass in her paws, as she won’t let me touch them, and she was home alone when it occured. She cannot reach the countertop where the dish was located and there were no items that had fallen on it. I decided to look at the reviews on Walmart and dozens of people have reported the same issue.

The product, in the report: Anchor Hocking glass cake baking dish, 8.5” X 8.5” Heavy, Clear glass, with handles on 2 sides along with an opening to hold it.
New Years Day ...I had 2 new Anchor Hocking glassware cake baking dishes in my oven set at 330 degrees. When I checked to see if it was done, I could not believe my eyes, total explosion on the top rack, glass and food everywhere. Chunks of glass and small pieces, like sand or dust. I was concerned that the other dish would also explode so I shut the oven and the door. Went to my party without my 2 casseroles. 2 days later I started to clean it up. I knew to protect my hands with gloves BUT I never expected my eyes to get injured! I was using a wet paper towel (w/water) to clean. Soon I was in pain, my eyes hurt. It felt like fine gravel or sand was in my eyes. If I wasn’t wearing my eyeglasses it would have hit both eyes worse, I hurried to the sink, flushing out my eyes over and over again. I looked up online about Anchor glass baking dish exploding in my oven. I was appalled that this has happened to many others! They called it glass dust or glass sand that injured their eyes like mine. I thought flushing my eyes would clean them out. My eyes were both red. within 24-30 hours my eyes were stuck together and when I gently separated them, it looked like nasty puss and extra red. A few hours later my eye doctor would be open. He fit me in. Thank God there were no pieces of glass in my eyes, just extremely red and injured. I used prescription eye drops for 2 weeks. I also went to my doctor’s office because of a cough and very hoarse voice yet I did not have a cold before or after this incident. My breathing seemed fine but she thought I should have an Xray to make sure my lungs were ok. I read up on this and learned it shouldn’t damage my lungs, only if it was prolonged inhaling or if someone worked under those conditions. About 6 -7 weeks later, my eyes are glassy. watery and a little red. I hope nothing worse happens in the future from the food and sand-like glass. I hope the infection is gone. I have many pictures of the shattered glass mess and my injured eyes which looked disgusting. There were black pieces in chunks, the edges of the food must have burnt in the explosion. I thought it was parts of my gas oven. Glass chunks were pushed right into the draw under the oven and on the sides going down to where the gas flames are beyond my reach. I have not used my oven sense. I plan on having my plumber check to see if it’s safe to use. When Anchor Hocking glass was safer, they used annealed borosilicate glass. They state the tempered soda lime glass is just as safe and has equal or better thermal shock resistance than the borosilicate glass. I do not trust this company! They care about impact resistance against drops or utensils chipping it. HOW about the people that get injured from it exploding, shattering so small you can’t see it. But cleaning the sink with a wet paper towel, I could hear the grit and feel sand like pieces all in the sink. It makes me wonder why they sold the company twice I believe. Did they already know about injuries and passed on the problem to the next owner! Too many only care about the money, the income, less cost ... but how about correcting this problem? I am so angry that this has happened to several people and yet they still sell this product. There should be a test of the older glass and this tempered soda lime glass. If we drop the dish on the floor, it’s our fault but when it explodes in a 330 degree oven this is the manufacturer’s fault. I really hope Consumer Reports can stop Anchor Hocking from causing more injuries to the consumer and CARE about others.

The product, in the report: 9X13 glass baking dish with a royal blue trimmed plastic cover
I used an Anchor Hocking 9X13 glass pan for a chicken artichoke and spinach recipe. Upon putting the dish in a 400 degree oven, within 10 minutes of cooking, the glass dish exploded in many pieces. All of the ingredients had been out of the refrigerator for just under 1 hour, so the dish was not too cold.

The product, in the report: https://www.walmart.com/ip/Anchor-Hocking-Oven-Basics-Tempered-Clear-Glass-Rectangular-Baking-5-Quart-Dish/28503419?classType=VARIANT&from=/search
I was cooking dinner using an Anchor Hocking Glass bakeware pan - we placed our food in the baking dish (the food was warm and the baking dish was room temperature). We put the baking dish in a preheated oven - 375 degrees. It was supposed to cook for 30 minutes. At 28 minutes the dish exploded in my oven. If it had been a couple of minutes later, my son would have been removing that dish from the oven. We checked the Walmart website (where I bought the dish) and found other reviews stating that this dish had exploded in other people’s stoves.

I had purchased an Anchor Hocking 2.75 qt. Rectangular TrueSeal Glass Food Storage 11 c. container several months ago. I was baking a salmon filet using the glass container when it exploded in the oven (375 degree) after 20 mins of baking. The glass container had been at room temperature prior to being placed in the pre-heated oven and did not have any obvious chips, cracks, or damage. Thankfully, the oven was closed when the glass container shattered, but I did cut my hand as I was picking the glass pieces up.

I did not sustain any injuries shockingly. Glass shards were stuck in my pants. The glass shards damaged the nearby cupboards by chipping and scratching the white paint.

The product, in the report: Anchor hocking blue glass 9x12 baking dish
I had a 9x12 Anchor Hocking baking dish in the oven. I removed it and placed it on the burners on my stove. The burners were turned on and were on a low setting. I was making gravy using flour and water, adding these to the drippings that were in the dish. This is something I have done with this dish for years. All of a sudden the dish exploded on the stove top and glass flew everywhere in my kitchen. As I was standing right over the dish when it exploded, I feel very fortunate that I was not injured in any way. Just scared out of my wits.

The product, in the report: Clear glass Anchor Hocking baking dish 13 x 9.
I placed an Anchor Hocking glass baking dish in the oven with 6 rolls to bake at 350 for 13 minutes. At around 12 minutes I peeked in the oven to see if they had browned yet, they had not so i closed the door and decided to wait until the 13 min mark about 2 minutes later I heard a loud sound from the oven and to my complete horror when I opened the oven door there were pieces of glass everywhere and still making sounds so i closed the door quickly and turned the oven off. If i had opened the oven door to check on the doneness of the rolls as it exploded, i can only imagine what could have happened, as i am a nurse and have seen a lot. This could have not only blinded me but also burned and possibly could have killed me in the case a shard had penetrated my carotid which is not out of the realm of posssibilities and if i had been taking it out and holding it something fatal could have happened and to my utter dismay find almost 17-18 pages on this site with complaints about the same Anchor Hocking glass baking dish that states that it is made for baking and sold as such. I am so lucky to not have been seriously injured or blinded or disfigured or burned. This is beyond my comprehension.

We had an Anchor glass loaf pan shatter in the dishwasher, even though it was listed and marked as dishwasher safe. The pan had been sitting on the counter until we consumed all the bread loaf we had made, which also meant there shouldn’t have been any known sudden thermal changes (except by the dishwasher cycle). Injuries could have occurred when cleaning up the shattered glass or if shattered glass had gotten in to other “clean” dishware.

We had purchased a Anchor Hocking 3.7 Qt Bake Deep Dish tempered glass cookware, stated to be Tempered Touch glass is oven, microwave, freezer and dishwasher safe to use to cook a roast on June 24, 2021 at our local Safeway. We cooked a roast at 375 for 1.5 hours, then removed the pan from the oven and placed it on the unlit burners on the stove in our kitchen to let it cool. We sat down and started eating, about 15 minutes into the meal, we heard a loud crash and heard a can get knocked off of the kitchen island. When I walked over to the island I saw glass everywhere on the island and surrounding floor. The glassware had literally exploded, shooting glass everywhere in the kitchen. As this was tempered glass, it was my understanding that this should not have happened, and the exploding glass was clearly dangerous and hazardous to clean up.

This happened on 5/22/2021 around 7:30 pm. Dish was less than 5 months old. Use about five to ten times only. Anchor Hocking Oven Basics 4.8-quart Glass Baking Dish, Rectangular, Set of 1, exploded in cabinet when lightly touched with another dish. EXPLODED!!! Shattered into a hundred sharp shards of glass. 70% of the shards traveled all directions in cabinet. 30% flew out towards me with great force. One sharp piece of glass impaled itself in the top of my foot at an angle making about a 1 inch cut and a puddle of blood. I had a few other tiny pieces of glass hit my feet in a few other places and had several small cuts on the top and sides of my feet. I needed a doctor. This was in a lower cabinet at floor level, and the force of the explosion sent the glass flying sideways. The dish was firmly resting on the shorter side on a thick rubber lining at the bottom of the cabinet. The glass was at room temperature, as was the glass next to it. I did NOT drop it or bang anything into it!!! I’m in my 50’s and cook all the time. I have never seen glass react like this before. So dangerous!!! Broken glass and blood everywhere!!!! What would have happened if this was a higher cabinet at face level???? I could have lost an eye! Please stop selling this product or other low quality dangerous glassware that is not tempered. Look on amazon for more reviews like this. This has happened to many other people too!!!!!!

I had no idea what it could have been so I opened the oven to discover that the glass baking dish had shattered into a million pieces...it did not just crack into a few pieces. The product, in the report: Anchor Hocking Company “4 pc bake n take” Clear Glass Oblong Baking Dish 3 quart Oblong sized 9 inch by 13 inch Oven and microwave safe up to 425 degree oven Model 4 pc bake n take
On Thursday, November 28, 2019, the Anchor Hocking glass baking dish I was using exploded and shattered sending glass flying further into my oven and all over my kitchen. The blast sent shards of glass into my face and all over the oven and kitchen floor. My oven was destroyed. I was baking sweet potatoes in the oven. I followed the recipe. It called for cubing the potatoes and baking them in a glass dish in a 400 degree oven, turning them a few times over the course of an hour. When I opened the oven for the second turning (with a wooden spoon), the glass baking dish spontaneously shattered and exploded. Glass shards were sent everywhere, including deeper within the oven, my kitchen, my wood floor, my kitchen mat. The heat of the blast hit my face and I fell back. I was not hurt, but was very upset and shocked. Glass fragments were everywhere. My oven was destroyed, glass melted into my floor, penetrated my oven mitts and slippers, and ruined a kitchen mat. My sweet potatoes were also ruined, so we did not have them at Thanksgiving. The Anchor Hocking Glass Baking dish was only 2 years old and it was the second time I had used it. It was clear glass, oblong, 3 qt capacity, 13 in by 9 in. There were no scratches or chips in the glass. It exploded spontaneously and without cause. I called Anchor Hocking to report the accident caused by the exploding glass baking dish on Monday, December 2, 2019. Anchor Hocking took down all my information and said they would get back to me in a day or 2. They never did. I called again on December 11, 2019 and December 16, 2019. I kept getting the run around. On December 19, 2019, I received and email from Anchor Hocking that my claim for a replacement oven was turned over to [REDACTED]. On January 2, 2020 an inspector from [REDACTED] came over to inspect the damage, collect the glass shards, take photos. On February 17, 2020, I received an email that my claim has been denied because Anchor Hocking/[REDACTED] found that the glass baking dish was NOT defective. I know it was defective and have read about similar incidences. I am opening this claim to push the company to accept responsibility for the exploding glass baking dish

The product, in the report: Glass food storage container with plastic lid
I purchased a set of Anchor Hocking glass food containers with blue plastic lids from Menard’s in St. Paul, MN this past summer (about two months ago). These are the kind of bowls that are meant for food storage and are generally used instead of plastic tupperware-style containers. This morning I placed frozen blueberries and refrigerated yogurt in the container and placed the lid on the bowl. I got to work about 40 minutes later, ate the yogurt (the blueberries had thawed), and left the empty bowl with the lid on it on my desk, about 12 inches away from me. About 15 minutes after I finished the food, I heard a loud pop and looked up to see that the glass bowl had exploded, spontaneously, sending glass shards to a distance of about three feet. I was not injured. I have a picture of the glass shards that remained on my desk. Please consider a safety recall before someone gets injured. I will be discarding the rest of the set immediately.

The product, in the report: Anchor Hocking Laurel Embossed clear pink tinted glass 8.25 x 8.25 baking dish
Anchor Hocking Laurel Embossed™ Collection bakeware 8X8 baking dish exploded in my oven. Prior to baking, the dish was room temp and the brownie batter was room temp. Per baking directions, I had greased the bottom inside the dish. The dish was filled with batter and placed in the center of the oven at 325 degrees for 50 minutes. About 40 minutes into baking, I heard a loud explosion sound but had no idea where it came from. A few seconds later, there was a second explosion. I realized it was coming from the oven and carefully peered in. The dish had completely exploded into millions of glass shards. I have checked online and found multiple reports of similar incidents. Even incidents where they have exploded in people’s hands while they were removing it from the oven. Thank God my 9 year old granddaughter was not removing it when this happened!

The product, in the report: Anchor Hocking Casserole 4 quart
I prepared lemon chicken as usual. I have made this dish for over 15 years the same way. I placed the chicken in the oven in an Anchor Hocking glass casserole dish. Five minutes later I heard an explosion in my kitchen and smoke began to fill the oven, kitchen, living room and family room. I immediately turned off my gas stove. When I opened the door of the oven, I saw that the glass casserole dish had exploded in my oven. There was olive oil and chicken broth and broken glass all over the bottom of my oven. The 5 pieces of chicken were sitting on the remainder of broken glass on the oven rack. This is a Fire Hazzard! I can only imagine if I had not been in a room close to the kitchen when this happened. My whole home could have burned down. Not to mention, if my teenager or any family member had removed this faulty casserole dish from the oven with the defect that HAS BEEN KNOWN to THIS company for years, a severe oil burn, or laceration could have happened. There have been many complaints regarding this same issue with Anchor Hocking. I find it unusual that there has been nothing done about these SAFETY issues with HUNDREDS of Complaints. ANCHOR HOCKING has explained the exploding glass as a safety mechanism or faulty care by the consumer. My casserole was less than 1 year old. There was NO extreme temperature changes and NO damage to my casserole prior to using. I now am afraid to use my Kitchen Aide oven that is saturated with oil near the gas igniter. I have saved the glass from the casserole dish for testing if needed. I also have many pictures.

The product, in the report: 8x8 inch square glass pan made by Anchor. 2 quart. Maybe 3 years old.
A glass 8x8 inch pan made by Anchor shattered. It was about 3 years old. I was cooking dinner in the oven at 350 degrees for 45 minutes. I took the pan out of the oven and was setting it on a potholder on the counter and it shattered into hundreds of pieces. I’m lucky I wasn’t hurt. This is extremely dangerous! I’ve been doing the exact same thing of setting a hot glass pan on a thick, dry potholder on the counter for 45 years. I will also mention that there were other dishes (REDACTED) and flatware on the counter. It is unlikely, but possible, that the pan touched something. Even if that were the case (and I don’t think it was), it should not explode. This glass should be recalled. It’s unsafe.

The product, in the report: Bake ‘N’ Store 5 cup baking dish with teal plastic and rubber lid
The consumer set the oven to bake at 350 degrees and placed the dish inside. The dish had bread stuffing. Approximately 10 minutes later, the consumer heard a loud explosion and noticed that the dish exploded. The consumer states that the dish was advertised as oven safe. The consumer indicated that this was the first time she used the dish. The dish shattered into pieces and the consumer threw away the pieces of glass.
The consumer then visited the firm’s website and filled out their contact form; advising them of the incident. The firm advised the consumer to send them the pieces of glass but she no longer had them. The consumer was told that the glass was made of soda-lime and would shatter into large pieces but the consumer stated that the pieces were both large and small. They also suggested that the dish could have been cracked prior to the incident but the consumer said it was not cracked. The consumer was also advised that the dish is discontinued but she is concerned that it is still in the market and it should be recalled.
The consumer also visited Safeway’s website and filled out their contact form; advising them of the incident. The consumer stated that she never received a response from Safeway.

Shattered Glass Baking Dish - Injuries, Damage
Dear Consumer Affairs Department:
On September 5th, 2016, I was using one of your glass baking dishes and it shattered spontaneously in my hands upon removing it from the oven. As I had heard of glass baking dishes shattering if placed on a stone counter from the oven, I did not set the dish on the counter top and I did not bump it in any way.
Due to the glass shattering, I received a burn and cut on my arm and a cut on my leg. Thank goodness I was wearing glasses and it was not one of my children who removed the dish!
In addition to suffering injury from the dish shattering, the shooting glass caused the following damage:
• Hot glass melted into my rug. • Glass shards scratched my wood flooring. • I lost a full dinner, which was contaminated with glass and had to be replaced with a dinner out. • The dish was destroyed. I have others that I will not use and must replace.
Attached are photos of the damage. Please contact me to discuss a resolution and what is being done by Anchor Hocking to make consumers aware of the possibility for your baking dishes to shatter. I see on your website that new products come with safety warnings, but that action does not educate consumers who already own your products. I would expect that Anchor Hocking would have in place a full consumer education campaign that includes public service ads and consumer segments on television. I will be filing a complaint with the Consumer Product Safety Commission.
Sincerely,
[REDACTED]
Director, [REDACTED]

We use Anchor Hocking Green TrueSeal containers in a variety of sizes from our local Container Store. Last night, while picking up a bowl, my finger slid across the bottom and this happened:
[REDACTED]
A shard of glass about 1/2” long broke loose and pierced my finger, going in one side and out the center. I’ve since read multiple reports of the same glass shards in Amazon reviews of the product.

The product, in the report: glass baking dish 7” x 9” or 7 x 11
The dish was in the oven for approximately 35 - 45 minutes. The oven was set to 350 degrees. The consumer went outside to do some yard work and when she returned, she noticed a burning odor. She checked the oven and found that there was a slit in the glass backing dish and some of the casserole had dripped from it down onto the heating element. The consumer turned the oven off and attempted to remove the dish. As soon as she touched the dish with her pot-holder, it began to shatter. The consumer had other baking dishes in the oven at the time and she said glass shards had flown from the cracked dish into the food of the other dishes that were in the oven at that time. As soon as she touched it with her pot-holder, the dish began to shatter even more. There were no injuries.
* The consumer threw away all of the food in the oven, because of the glass in the other dishes.
11/30/2015. The consumer called the mfr. to discuss this matter and their rep. referred her to CPSC to file a report.
The consumer said that she believes that this was only her 2nd time using this smaller baking dish. It came in a set of 2 and the second dish was approx. 9” x 13”, but it was not damaged. The consumer said that she saw several other reports of this type of problem online and she questions why this has not been addressed or corrected.

I was baking a boxed pumpkin quick bread in my Anchor Hocking glass loaf pan. The oven was at 350 and baked @50 min....as per pkg directions. When the bread was done we took it out and placed it on top of the stove to cool. None of the burners were...or had been on. The stove was warm from baking the bread but that was all. We heard 3 loud noises from the kitchen and the glass had shattered everywhere. I’m so glad we were not by it at the time! I have 2 children and I would hate to think about them getting hurt! It shattered with such force that glass was found in the other room.....more than 10 feet away!! This is NOT acceptable!! Please handle this so no one gets hurt in the future!! Thank you, [REDACTED]

The product, in the report: Anchor Hocking glass baking dish.
This evening I had a large glassware baking pan explode in my kitchen upon removal from the oven. This baking dish exploded in my boyfriend’s hands leaving glass chunks, shards, slivers and dust all over my kitchen- stovetop, inside of oven door and oven, countertops and floor.
The Anchor Hocking website, on the Bakeware Facts page, states: ... these pieces generally lack sharp edges when it does break, resulting in a lower likelihood of severe cuts from the broken glass.
Likewise, having the bakeware break in a safer manner by breaking into pieces generally lacking sharp edges and shards is a priority.
The bake ware broke into pieces and shards that did contain sharp edges- some pieces were large, long, and sharp. Other pieces were very small and sharp, including a glass dust that I had to wipe and vaccum surfaces several times to remove. I am still not confident that I can safely walk barefoot in my kitchen or use my countertops for food preparation.
We have owned this bakeware for about three years, and have always followed the safety instructions during use.
My family is on a limited budget- I had to throw away the food we were preparing. This product caused an inconvenience, as far as dinner was concerned. But far more concerning is the safety issue this product poses. Luckily my boyfriend has quick reflexes and did not sustain any physical injuries.
I have sent an email to consumer@anchorhocking.com with this same description of the incident

The product, in the report: Anchor glass casserole dish.
April 10, 2015
To Whom It May Concern: In 2014 we purchased an Anchor value pack of 3 glass casseroles from Walmart. Last night (4/9/15) at approximately 5:30PM, we were cooking Cornish Hens in the 5 quart glass casserole. Half way through the cooking process, the glass casserole exploded inside the oven, seconds after I had been attending to the hens. I was cooking the hens according to the directions of a recipe I’ve cooked several times before. We stuffed the hens with rosemary & garlic and cooked them in a 450 degree oven for 25 minutes, then lowered the temperature to 350 degrees and slowly poured 1/3 cup of white wine & 1/3 cup of chicken stock over the hens. Both of these ingredients were at room temperature and I had opened the oven door to lower the inside temperature before slowly pouring the wine/stock mix over the hens. All 3 of the glass casseroles as well as the packaging state the casseroles are safe for conventional ovens which is how we were using it. The clean-up process was horrid, glass and shards were everywhere. We had to use a shop vacuum to clean out the inside of the oven and the surrounding areas. We discovered that the oven door was hit so hard by flying glass that the door now has a vapor leak and the door will need to be replaced. The hardwood floor in front of the oven is a mess. Thank God I had my reading glasses on or I might have received serious injuries to my face not just cuts to my hand or my foot. Needless to say I was a nervous wreck and jumpy the entire night. Even today, thinking about how I could have been seriously injured or maimed, I remain very upset and concerned for others. This morning I googled Anchor glass explosions and discovered that this is a long standing problem with these glass casseroles. Why would you allow these items and packaging to still say safe for conventional ovens with no warnings? I also question Walmart selling this product. What about consumer protection? We have photos of my cuts and the shattered glass casserole. We kept the glass pieces from the shop vacuum for possible further analysis, we have a video of the inside of the oven after the explosion, we have a picture of the temperature of the stove just after the explosion and we have the remaining 2 casseroles from the value pack. We will notify Walmart of the explosion and our concerns for consumer protection. Please contact us if additional information is needed. {REDACTED]

I had a glass lid explode in my microwave. I was steaming cut vegetables with a couple tablespoons of water. Room temperature veggies, water from the faucet and placed in the microwave. It had been on high for about 2 minutes when I heard an extremely loud bang.
Attached are pictures of what the inside of the microwave looked like.
This glassware is almost brand new. Bought after Thanksgiving (could’ve been after Christmas) at Target as a three pack. Has been used in the oven not more than 3 times total (over the three dishes), always at less than 400 F. Used in the microwave at most a couple times a week (again over the three dishes). They were always washed in the dishwasher.

The product, in the report: Anchor Hocking clear glass baking pan 9 x 13 in size.
I am reporting this incident for my son, [REDACTED]. The reason I am reporting this is because I purchased a three piece baking set of Anchor Hocking glass baking pans for him for Christmas in either 2011 or 2012. I am mortified that the product I purchased for him was so dangerous and could possibly cause very dangerous life threatening injuries. Actually after this incident, I did some research about these products and found out that there have been many incidents with this same product that have caused very serious bodily injuries to innocent consumers.
He was baking sausage in an Anchor Hocking glass baking dish, sized 9 x 13, and when he took it out of the oven and put it down on the stove for approximately one minute and he took the sausage out and then walked to the sink and then it blew up as he went to set it down. The pan exploded into many, many shards of glass. Glass shot all around the room, into the sink, all over the counters and even some of the larger glass shards shot across the room, at least 7 - 10 feet.
Needless to say, my son was completely terrified by this event. It took him hours to calm down. It was as if he was in a war zone and a bomb went off, the only difference, and it is a BIG difference, was he was in his own home, in his own kitchen where he should feel safe and sound.
He and his wife gathered and saved the glass from the pan that exploded for evidence. I also have photographs of the aftermath. I am in hopes that your agency can help direct up where to go with this issue and to help prevent this from happening to other innocent consumers.
My son also lost food that he was preparing for his nightly dinner. I feel he should be compensated for the set of pans (three) total, the food cost and something to compensate him for the stress he underwent that night.

I am writing at the urging of a friend when she saw my photos of my injury. I have photos of oven with second baking dish intact, photos of injuries on leg, photos of oven model, photos of clothing through which glass penetrated, and photos of mess on floor. I will be glad to send.to the appropriate person. (I could not upload them from my [REDACTED] but could send them in an email.)
This incident with exploding glass happened in September when I was canning tomatoes. I roast them in the oven (350 degrees) then carry the baking pan with roasted tomatoes to canning jars approximately four feet away. I have been doing this for at least 10 years with the same three pans: one a metal oven pan, one a 50 year old glass [REDACTED] baking dish, and one a recent Anchor Hocking glass baking dish.
As I lifted (with pot holders) the Anchor Hocking glass baking dish from the oven and began a turn toward the counter with canning jars, the dish exploded in my hands. I am extremely fortunate I was slightly bent and the pan was in a low position at this moment--and extremely fortunate that my grandchildren were not helping at this time!
I called Anchor Hocking and they offered to replace with more baking dishes. Why would I want more of their dishes? I have a second Anchor Hocking glass dish, smaller than the one that exploded. The two were purchased as a set so I was able to identify the brand of the exploding dish. Because we were leaving on a trip and I was able to nurse my own wounds with antiseptic and bandages. I went on with my life. Now a friend happened to look through my pictures and was appalled that I had not informed the government of the extreme safety issue. And, reading on the Internet I see it is not an uncommon event. The pictures will tell the whole story if anyone is interested. I am sorry I didn’t seek medical attention because then it would have been reported at the time and Anchor Hocking would have had to pay something for my medical and hopefully would have had to answer to the government.

Anchor Hocking baking dish exploded in my mother’s oven on Sunday, Jan. 12, 2014. My mother was fixing meatloaf, potatoes, and carrots in a baking dish recently purchased at Wal Mart in Sidney, Ohio. My mother and I have used this bake ware for decades never having a problem with how we used it. We did not know of the changes to the newer product that altered how to use it. We have always been able to put it in the oven and turn it on or program for starting to cook later, with every expectation that nothing would go wrong, it never had before. The baking dish was put in the over then was lit to start heating, minutes later it exploded in the oven. The food inside was still cold, the meat raw not even beginning to get warm. The sound and the was horrendous! After this incident I researched the problem to see what possibly could have caused it. Then I discovered the advice to always put in preheated oven, not a former requirement of the old products. This means this is no longer a safe product to use in programmable ovens where it starts cooking when no one is home. It is difficult for the elderly who have used a product the same way for decades to realize the product has changed, that they are put at risk if their habits are no longer applicable to the new lines of products.

The product, in the report: 9x13 Anchor Hocking blue glass casserole pan
I cooked a taco pie casserole in my blue Anchor Hocking 9x13 baking dish. When I took it out of the oven and placed it on my smoothe surface cooktop (not heated), the dish blew up and sent glass 10 feet across the room. I had 28 people in my house for a Christmas party. Glass was everywhere. The food was ruined.

The product, in the report: Glass Bakeware
Caller was taking his 9 x 13 glass bake-ware from the shelf when it exploded in his hands.
He had not been baking in the dish and hadn’t baked in it since Thanksgiving which was a month before this report.
The farthest away that the glass from the dish went is four feet and the nearest is inside the kitchen cabinet he was removing it from. He still has the shards from the bake-ware.
The caller’s palm of his left hand has a superficial 1/8” cut with some bleeding which he has treated himself.
He has not contacted the manufacturer and doesn’t know whether he can or not because he has two manufacturer’s glass baking dishes and isn’t sure which manufacturer this particular dish came from because it is now unidentifiable.

The product, in the report: Anchor Hocking 9 inch Pieplates (2 Pack) Model number 67301L11; UPC code 0 76440 67301 2
I purchased a 2-pack of Anchor Hocking Glass Pieplates model # 67301L11 (UPC Code 0 76440 67301 2) and one of the plates spontaneously exploded when I picked it up. The pieplate was not exposed to either heat or cold at the time of the incident - I was washing the product with lukewarm water. The glass shards casued a serious laceration on my thumb including a severing of the tendon. I needed hand surgery to repair the tendon and will likely need multiple months of hand therapy in the future. I currently have no use of my right thumb.

The product, in the report: 9*13 glass pan
My children and I were eating dinner at the table when a glass dish from anchor blew up all over my son the table and the floor. Thankfully the glass did not injure anyone. we did have to throw away food but that is not the point. the point is that if just one of those pieces would have hit any of my children the right way they would have been injured. I checked the shards they were not hot or even warm to the touch. It randomly blew up.

The product, in the report: 9x13 anchor glass baking dish
We placed a room temperature 9x13 glass baking pan on top of another glass baking dish which was also room temp. In a few minutes we heard a loud bang and found thousands of pieces of glass all over the kitchen and 8 feet down the hall. The pan had exploded. Thankfully, my husband who was sitting at the kitchen table was unharmed and my 2 year old daughter who was walking down the hall was unharmed. We thank God for His protection!!! There was not any large pieces of glass remaining from the pan only small pieces that were everywhere. We were shocked!!!

The product, in the report: Anchor Hocking 10x15” (approx.) -- 1 size up from 9x13” -- clear glass rectangular baking dish. Came with green plastic storage or travel lid.
I had an Anchor Hocking glass rectangular dish approximately 10x15 -- one size larger than 9x13” -- for several years. No apparent cracks or chips on it. It had not been used in a couple months. I reached into my cabinet and put my hand on the handle to lift it up and out (but had not done so yet) and it shattered into hundreds of pieces flying everywhere. I only ended up with one small cut on my hand, but this was the most bizarre thing I have ever experienced.

The product, in the report: Approx. 2 qt. glass casserole dish, rectangular
I had a glass casserole dish explode on Monday night, May 27. The dish was given to me probably more than 10 years ago and was part of a set. The dish which exploded was approximately a 2 quart, rectangular dish, with sides about 2 inches high. It exploded inside the oven and no one was harmed. The events related to this are below: Earlier in the evening I removed a stick of frozen butter and put it in the dish on the counter. It probably stayed there 30-60 minutes. I pulled another item out of the oven (thinking it was done) and placed the dish with the butter in the oven (to melt the butter). The oven was probably set at 350-360 degrees. After a few minutes, I realized that the other item was not done, so I removed the dish with the butter and set it on the (glass) stovetop (which was warm to the touch, but one could leave his hand on it and not be uncomfortable). After maybe 10 minutes, I removed the other item from the oven and returned the dish with the butter back in the oven to finish melting the butter. When I came back a few minutes later and checked the dish in the oven with the butter, it had exploded with glass pieces (and butter) all over the oven. I turned off the oven, left it closed, and allowed it to cool before attempting to remove the glass pieces and clean. After it had cooled, I collected the glass pieces and cleaned the oven. I have the glass pieces and paper towels used to clean the oven in a heavy trash bag. The oven has been fully cleaned and vacuumed to remove any remaining small pieces of glass.

The product, in the report: Clear glass heavy duty casserole dish from Anchor Hocking.
On Tuesday evening July 25th, 2013 I was cooking with a new Anchor Hocking glass casserole dish and had it in the oven for 25 minutes at a temperature of 350 degrees Fahrenheit. The dish belonged to my sister-in-law who got it as a wedding gift in January. It was during the time that I was checking to see that the chicken had cooked all the way through that the glass dish suddenly exploded with a loud “pop” and sent bits and large shards of glass all over the kitchen. One of these shards, approximately 8” in length flew off of the table and imbedded itself in the top of my left foot. After removing the shard an approximate “dime-sized” hole was left in my foot and I had no use of two of my toes. The wound was treated with thorough cleaning, compression, and medical glue. The incident has left me on crutches for a, nurse practitioner recommended, week.

The product, in the report: Anchor [glass] pan approx. 10x13, and loaf size pan, also Anchor [glass]
A [glass] baking dish was pulled out of the oven and placed on a warm stove. It appeared fine for a couple minutes until it exploded all over the kitchen and well into the living room at the children’s group home that I work in. This has since happened with a smaller baking dish that was simply removed from the oven, made a terrible noise, and then just popped/broke. It was a loaf sized pan as opposed to a full size baking dish.

The product, in the report: 8x8x2.25in 2qt clear glass baking dish
While cooking scalloped potatoes in my gas oven, the Anchor Hocking clear glass 8x8 2qt cooking dish shattered into more than 50 pieces and causing the door to my oven to slightly jar open, make a very loud sound, and scattering the contents of the pan all over the oven. The oven was preheated to 425F, the dish was in the oven approximately 12 minutes into a 25 minute cooking time. I have owned the cookware piece approximately 3 years and have used it for this exact purpose more than 20 times without incident prior to the sudden shattering.

Emergency room Made Apr 7, 2013, sent to the company Apr 15, 2013, published by CPSC May 6, 2013
The product, in the report: oven safe baking pan typically referred to as [REDACTED]
last night (saturday) i had a chicken in a medium sized baking dish made by Anchor Hocking in the oven at 425. (i believe it is Anchor Hocking because the other dish that i bought at the same time was Anchor Hocking). I did also retain some of the shattered glass.
i incorrectly then transferred the baking dish to the stove top grill and did again misuse the product by applying heat to it. the glass exploded sending shards as far as 6 feet away embedding into my clothing. we threw out all permeable items from kitchen area. i took pictures and kept some of the glass. 2 hours later i noticed my eye was irritated. i went to ER to have it checked out. the ER doc numbed my right eye, with [REDACTED] and used fluorescein to help look. he removed something “clear” from the white area and also had me xrayed. he reported seeing a scratch. i will try to make an appointment with an eye doctor monday.
i am 46 years old and experienced in the kitchen. if i could make this error in judgement and put my eyesight at risk, surely others could as well. i believe the company should make the stovetop use warning more pronounced.
