What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Supervision of hospice aides (42 CFR 418.76(h)(2))

    The inspectors’ words, 268 words

“It is required that a registered nurse makes an annual on-site visit to the location where a patient is receiving care in order to observe and assess each hospice aide while he or she is performing care per the hospice’s policies and procedures. This standard was not met as evidenced by document review, interview, and personnel record review. Agency policy #1-036.1 “Hospice Aide Training” (dated 05/11/2020), read in part, “Annual performance reviews...will include on-site skills assessments for supervisory visits done annually...” In 1 of 1 (100%) personnel record review of a Hospice Aide, the agency did not ensure that the Registered Nurse made an annual onsite visit to the location where the patient was receiving care to observe the aide performing care. Personnel Record 9: The Hospice Aide was hired on 09/12/2016. A personnel record review on 05/29/2025 revealed no evidence of an annual on-site skills assessment supervisory visit for 2023. During a joint clinical record review and interview on 05/30/2025, the Regional Director of Clinical Operations/Acting Director of Clinical Services and Administrator confirmed there was no documentation found in the clinical records of the Registered Nurse performing aide supervision while the aide was present. During an interview on 05/29/2025, the Regional Director of Human Relations confirmed that the only annual documentation located in the personnel record for the Hospice Aide (HA) for 2023 was the Annual Performance Review and the “Annual Competency Skills Checklist Review,” which only included the attestation statement, “This is to certify that I, (name), have reviewed my competency skills checklist.” It was signed and dated by the HA and the RN on 08/24/2023.”

What do families say?

  • CMS gives no family caregiver survey rating: “The number of cases is too small to report Star Ratings.”
  • In CMS’s survey of family caregivers (Oct 1, 2023 to Sep 30, 2025): 83% rated it 9 or 10 out of 10, and 88% would definitely recommend it.
  • CMS’s Hospice Care Index: 9 of 10 (Jan 1, 2023 to Dec 31, 2024).
  • 99.22% of patients were assessed on all seven of CMS’s measures at the start of care (Oct 1, 2024 to Sep 30, 2025); 67.7% had visits from a nurse or social worker on at least 2 of their last 3 days (Jan 1, 2023 to Dec 31, 2024).

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

From the hospice

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
