What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of comprehensive assessment (42 CFR 418.54(c)(6)(i))
The inspectors’ words, 228 words
“It is required that the documented comprehensive assessment includes a drug profile that contains the patient’s current prescription and over-the-counter (OTC) drugs, supplements, herbal remedies, and other alternative treatments that could affect drug therapy. The medication review process includes the identification of any drug therapy associated with laboratory monitoring. The drug profile is updated per the hospice policy and procedure, but, at minimum, at the time of each comprehensive assessment and/or when new medication is added or changes are made to existing medication. This standard was not met as evidenced by clinical record review, interview and observation. In 1 of 5 (20%) clinical records reviewed, the agency did not ensure the drug profile was updated to include all medications. Clinical Record 2: During the home visit (Home Visit 1) with the Registered Nurse and Director of Patient Care Services on 09/25/2024, a medication review was completed by comparing the hospice medication profile 08/09/2024-09/25/2024 to the facility medication list printed on 09/25/2024. The nursing facility medication list revealed the patient was taking Mirtazapine 15mg once daily effective 08/09/2024, multi-vitamin once daily, Acetaminophen 325mg two tablets once daily, and Sertraline 50mg once daily. The hospice medication profile between 08/09/2024-09/25/2024 did not include Mirtazapine, Multi-Vitamin, Acetaminophen, and Sertraline. During a joint record review and interview on 09/25/2024, the Director of Patient Care Services agreed that the drug profile was not updated.”

The patient’s diagnosis was Hypertensive Heart and Chronic Kidney Failure. During the home visit all of the medication the patient was currently taking was reviewed. The patient was taking Probiotic 1 tablet daily and Vitamin D3 10,000IU daily. Neither of these over-the-counter medications were listed on the medication profile. The patient stopped taking Cytomel 5mcg 2 tablets daily and Ezetimibe 10mg daily. Both of these medications were still listed as active medications on the medication profile. During the interview with the patient’s daughter, she stated that the patient was taking Escitalopram 10mg daily, Clonazepam 0.5mg daily, and Dicyclomine 10mg every 6 hours as needed for stomach pain. None of these medications were listed under active medications for the patient.
