What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 512 words
“Based on record review and interview the agency failed to ensure the patient clinical record included all clinical notes for 1 of 3 patients (Patient #1) for which records were reviewed. Failure of RN #1 to document skilled nurse visits and coordinate patient care resulted in members of the patient’s care team not being aware of the patient’s declining health status. Findings:Review of the agency’s policies and procedures revealed a policy titled “Medical Record Content IM.30, Reviewed:090124”, which read in part “The Hospice will initiate and maintain an individual and accurate medical record containing past and current information for each patient accepted for care. The record will contain correct clinical information that is available to the patient’s attending physician and hospice staff. The medical record will be in compliance with all federal and state laws and regulations. The content of the medical record will include at a minimum, Clinical notes; Documentation of communication for continuity of care.” Review of the clinical record for Patient #1 revealed a start of care date of 07/17/2024 diagnosis of Alzheimer’s Disease (decreased brain function). Review of the plan of care for the time period 05/13/2025 – 07/11/2025 revealed orders: SN- 2WK9 (2 times per week for 9 weeks) + 3PRN (as needed) for symptom, medication and pain management. Review of a document titled Client Coordination Note Report dated 06/19/2025. The document read in part, “Note: Chief Complaint: 6/19/2025 1914 (7:14 p.m.)- Patient’s (FM) is calling to state patient has been vomiting since 1830 (6:30 p.m.) today. Assessment: Patient is having complaints of abdominal pain. Patient does not have any medications for nausea or vomiting. Response: Triage nurse will contact nurse on call to follow up. Education: Triage nurse notified nurse on call, RN #1 for follow up.” Review of the Client Calendar Report revealed no visit was conducted on 06/19/2025. There was no documentation that RN #1 followed up with FM or contacted the physician to report the patient was having nausea and vomiting and obtain an order for medication. A telephone interview was conducted on 02/25/2026 at 11:10 a.m. with RN #1. Surveyor asked if he recalled receiving an after hours call on the evening of 06/19/2025 to report a change in condition of Patient #1 and a request for medications for nausea and vomiting. RN #1 stated he did not recall receiving a call. Surveyor asked if he contacted FM #1 or made a prn visit to assess Patient #1. RN #1 stated, “I don’t remember.” Surveyor advised there was no documentation of a SN visit or coordination with the physician on 06/19/2025. RN #1 stated he conducted a visit on the afternoon of 06/20/2025 after the family requested a second nurse come to assess the patient. Surveyor advised there was no documentation in the clinical record that he made a SN visit on 06/20/2025 and asked if RN #1 documented the visit. RN #1 stated, “If it’s not in the record, I guess not.” Surveyor asked why he didn’t document a visit note. RN #1 stated, “I don’t know. I must have forgotten.””
