What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of comprehensive assessment (42 CFR 418.54(c)(6)(i))
The inspectors’ words, 430 words
“It is required that the documented comprehensive assessment includes a drug profile that contains the patient’s current: prescription and over the counter (OTC) drugs, supplements, herbal remedies, other alternative treatments that could affect drug therapy. The medication review process includes the identification of the following: the effectiveness of drug therapy; drug side effects; actual or potential drug interactions; duplicate drug therapy; and drug therapy associated with laboratory monitoring. The assessment includes evidence that common side effects of medication in the hospice populations are anticipated, and as appropriate, preventive measurements are implemented to manage the side effects. The drug profile is updated per the hospice policy and procedure, but, at minimum, at the time of each comprehensive assessment and/or when new medication is added or changes are made to existing medication. This standard was not met as evidenced by document review, interview, and observation. In 2 of 3 (67%) home visits observed, the agency did not ensure an accurate review of the medications the patient was currently using to identify any potential adverse effects and drug reactions. DOCUMENT 2: The agency’s policy 11.10.1 titled Medication Profile, dated June 2019, required in part, The nurse will add all new/changed medication(s) to the medication profile. The medication profile will be reviewed every home visit, updated as changes occur, and reviewed by the IDG (interdisciplinary group). HOME VISIT 1: The patient’s representative elected the hospice benefit on 06/14/2024. The current medication profile was dated 05/21/2026, as part of the updated plan of care. During the home visit with the Registered Nurse (RN) (Personnel Record 1) on 05/28/2026, a review of the patient’s current home medications compared to the medication profile found a prescription filled on 10/14/2025 (Gas Relief 125 mg chewable tablet, one by mouth after each meal for gastritis and bloating), which was not listed on the medication profile. HOME VISIT 2: The patient’s representative elected the hospice benefit on 01/17/2025. The medication profile was dated 05/2/2026, as part of the updated plan of care. During the home visit with the hospice aide (Personnel Record 2) on 05/28/2026., a review of the patient’s current home medications compared to the medication profile found a prescription filled on 04/07/2026 (Nystatin 100,000 units/gram cream applied three times a day for fungal infection), which was not listed on the medication profile. An interview was conducted on 05/28/2026, with the Director of Nursing (Personnel Record 1) and Alternate Administrator (Personnel Record 8) regarding the findings of patients’ medication omitted from the agency’s medication profile. They stated that this issue is one of the agency’s active performance improvement projects.”
