What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Medical social services (42 CFR 418.64(c))

    The inspectors’ words, 213 words

“It is required that medical social services are provided by a qualified social worker per the plan of care; This standard was not met as evidenced by clinical record review and interview. In 2 of 19 (11%) clinical record review of patients that had social worker services revealed that the agency did not ensure the social worker provided care as ordered on the plan of care. CLINICAL RECORD 8: The clinical record review lacked evidence that the social worker was following the frequency ordered on the plan of care. During the benefit period from 02/09/2025 to 04/09/2025, the social worker documented patient visits twice in March (03/10/1015 and 03/18/2025). However, the care plan specified that the frequency order was once a month. During the benefit period from 01/23/2025 to 04/22/2025, the social worker documented patient visits twice in February (02/04/2025 and 02/05/2025). However, the care plan specified that the frequency was once a month. The record contained documentation of bereavement activity but no bereavement plan of care specify the services offered or the frequency of those services. CLINICAL RECORD 17: The hospice benefit was elected on 04/22/2025 and date of death 05/01/2025. The clinical record review lacked evidence of a bereavement plan of care. The record contained documentation of bereavement activity but no bereavement plan of care specify the services offered or the frequency of those services. During an interview on 09/24/2025, the administrator (Personnel Record 7) stated that they changed to a new EMR in May 2025 and are learning the system, but they will be doing additional education to review how to add the bereavement plan of care.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.
