What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 268 words
“Based on record review and interview, the agency failed to provide volunteer services in an amount that equals 5 percent of the total patient care hours of all paid hospice employees for one of the three years that records were reviewed (2024). This failure could place Agency clients at risk of inadequate care by not ensuring that Agency’s volunteers are being adequately staffed for hospice patients. Findings included: During a record review of the agencies policies on 7/24/2025, revealed no volunteer information had been found. Further record review revealed the agency had one continuous care (24-hour care) patient from the dates 6/6/2024-6/9/2024 equaling a total of 72 patient care hours for 2024. In an interview with the Administrator on 7/25/2024 at 12:00 p.m., the Administrator stated that the agency had no volunteer program due to the census currently 0. The Administrator further revealed that she has not had a volunteer in 5 years. The Administrator stated that if patient hours were obtained in 2024, that 5% (3 hours) cost savings of volunteer hours should have been utilized while servicing one patient in 2024. The Administrator also stated that if patient hours were obtained, that 5% cost savings of volunteer hours should have been utilized with efforts of active recruitment that should have been documented. Record review of the Agency’s policy titled, “Volunteers” dated 5/1/2020, revealed in part, “I. The Agency will provide volunteer services to be used in day-to-day administrative and/or direct patient care roles.” Further record review of the Agency’s policy revealed,” The hospice will document and demonstrate viable and ongoing efforts to recruit and retain volunteers.””
