What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of plan of care (42 CFR 418.56(c))
The inspectors’ words, 264 words
“Upon patient record review, 2 of 14 (Patients #2 and 8) did not contain evidence that the Hospice had developed an individualized written Plan of Care for each patient. The Plan of Care must reflect patient/family goals and interventions based on the problems identified in the initial, comprehensive, and updated comprehensive assessments. The Plan of Care must include all services necessary for the palliation and management of the terminal illness and related conditions: Patient #2: During the 12/17/24 surveyor-attended home visit the following was noted: 1) There was a sign on the patient’s wall that the patient has a right elbow splint, that must be in place whenever the patient is sitting in his Broda Chair; 2) The patient has two different right-hand splints, one to be applied during the daytime, and a different one to be applied over night-time; and 3) The patient had bilateral heel protector/boots. All of these DME were listed in the POC under DME. However, there were no details within the body of the POC regarding their correct application/frequency/duration, responsible personnel, etc... Patient #8: The Physician CTI dated 11/11/24 included, “Patient has a Vascular Boot on RLE.” This supply is not included on the most recent IDG dated 12/03/24, nor any details such as when it should be in place, who is responsible for applying, etc... The same IDG note has the RN documenting that, “Patient/caregiver has been applying Compression Stocking for 25” without result.” This is not a complete order, as it is not clear as to when the stockings should be applied/removed, nor by whom, etc ...”

Content of plan of care (42 CFR 418.56(c)(4))
The inspectors’ words, 203 words
“Upon patient record review, 1 of 14 (Patient #4) did not contain evidence that the Plan of Care included all services necessary for the palliation and management of the terminal illness and related conditions, including medications and treatments: Patient #4: The 11/22/24 IDG meeting notes reflected that the patient had bumped her calf resulting in a skin tear. The RN made a visit on 11/23/24 resulting in, “Daughter taught wound care, as follows: Remove old dressing by soaking with warm water, cleanse with Wound Cleanser, apply Triple Antibiotic, non-stick gauze, and secure with Koban. Change every other day.” A subsequent verbal order dated 11/26/24 included, “Cleanse with Wound Cleanser, pat dry, apply Xeroform to wound, Kerlix, then tape.” This is not a complete treatment order, as it does not clarify where on the body this wound treatment is intended, who is responsible for providing such wound care, nor a frequency. A subsequent verbal order dated 12/03/24 included, “Spray wound with Wound Cleanser, pat dry, apply Xeroform, Kerlix, tape, then ACE Wrap.” This is not a complete treatment order, as it does not clarify where on the body this wound treatment is intended, who is responsible for providing such wound care, nor a frequency.”

- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
