What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Content of comprehensive assessment (42 CFR 418.54(c)(6)(i))

    The inspectors’ words, 275 words

“It is required that the documented comprehensive assessment includes a drug profile that contains the patient’s current prescription and over-the-counter (OTC) drugs; supplements; Herbal remedies; Other alternative treatments that could affect drug therapy. The drug profile is updated per the hospice policy and procedure, but, at minimum, at the time of each comprehensive assessment and/or when new medication is added or changes are made to existing medication. This standard was not met as evidenced by clinical record review, interview and observation. In 1 of 14 (7%) clinical records reviewed it was revealed that the hospice did not ensure the comprehensive assessment included an updated medication profile that contained the patient’s current medications. CLINICAL RECORD 2/HOME VISIT 3: The patient elected the hospice benefit on 05/08/2026. The patient resided in a Skilled Nursing Facility (SNF). During the home visit with the Registered Nurse/Alternate Director Clinical Services (Personnel Record 2) on 06/10/2026, the patient’s medications were compared against the SNF active medication orders. The facility medication profile included: Hydroxyzine Pamoate 50mg one tablet twice daily starting 05/08/2026, Pantoprazole Sodium 40mg once daily starting 04/21/2026, Remeron 15mg one tablet at bedtime starting 04/17/2026, Thiamine 100mg one tablet daily starting 04/21/2026. These medications were not listed on the hospice medication profile between 05/08/2026 and 06/09/2026. In an interview with the Registered Nurse/Alternate Director Clinical Services (ADCS)(Personnel Record 2) it was confirmed that the medications were not listed on the hospice medication profile. Additionally, the RN/ADCS stated, “I didn’t have a med list to reconcile the meds until last Friday, June 5. It’s a new facility to us and I have had a hard time getting the med list printed.””

    Rule L0530. Standard-level.

  • Level of activity (42 CFR 418.78(e))

    The inspectors’ words, 269 words

“It is required that hospice volunteers provide day-to-day administrative or direct patient care services in an amount that, at a minimum, equals five percent (5%) of the total patient care hours of all paid hospice employees and contract staff. This standard was not met as evidenced by document review and interview. A review of the Volunteer Hours Reports revealed that the agency did not ensure volunteers provided administrative or direct patient care services at least 5% of the total patient care hours of all paid hospice employees and contracted staff. DOCUMENT 1: The Volunteer Requirement/Cost Savings Report for the time period 06/01/2025-05/31/2026 revealed volunteer hours provided 2.53% of the total patient care hours of all paid hospice employees and contract staff. A review of the Volunteer Requirement/Cost Savings Report for the time period 01/01/2025-12/31/2025 revealed volunteer hours provided 1.35% of the total patient care hours of all paid hospice employees and contract staff. During a joint review of the documentation and interview on 06/11/2026, the Volunteer Coordinator agreed there was no documented evidence that the volunteer hours equaled at least 5% of the total patient care hours. Additionally, the Volunteer Coordinator stated, “It has been a struggle to get more volunteers.” During a joint review of the documentation and interview on 06/11/2026, the Administrator confirmed that a performance improvement plan was started in December 2025 and is still in place. A review of the Volunteer Hours Reports revealed that the agency did not ensure volunteers provided administrative or direct patient care services at least 5% of the total patient care hours of all paid hospice employees and contracted staff.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
