What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Hospice aide assignments and duties (42 CFR 418.76(g)(4))
The inspectors’ words, 252 words
“It is required that Hospice aides must report changes in the patient’s medical, nursing, rehabilitative, and/or social needs to a registered nurse as the changes relate to the plan of care and any quality assessment and improvement activities. The hospice aide must also complete appropriate records of service, including the report of changes in the patient’s needs. Hospice aide documentation complies with hospice policies and procedures. This standard was not met, as evidenced by clinical record review and interview. In 1 of 9 (11%) clinical records reviewed for the patients who received Hospice Aide (HA) services and had a change related to the plan of care, the agency did not ensure the HA reported the changes in the patient’s medical needs to the Registered Nurse (RN). Clinical Record 2: The patient elected hospice care on 05/31/2024 with a diagnosis of other sequelae of cerebral infarction. The patient resided in a skilled nursing facility (SNF). The HA care plan dated 09/06/2024 assigned the HA the relevant task of: “Assist with feeding” every visit. The HA visit note dated 09/23/2024 documented, “Assist with feeding...Task already completed.” A clinical record review revealed no evidence that the HA notified the RN of the changes in the patient’s needs related to the HA care plan. During an interview on 10/04/2024, the Vice President of Clinical Operations, Regional Director of Clinical Operations/ Alternate Director of Patient Care Services, and Team Director stated that the Hospice Aide had not reported the change in the patient’s needs to the RN.”
