What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Counseling services (42 CFR 418.64(d)(1))

    The inspectors’ words, 238 words

“Based on review of the policy titled, “Bereavement Services,” clinical record review, and staff interview, it was determined the agency failed to establish a bereavement plan of care to reflect the needs of the bereaved for three patients (P) (P9, P10, P14) of three sampled bereavement charts. Findings included: Review of the undated policy titled “Bereavement Services” revealed the following: PROCEDUREAn initial Bereavement Risk Assessment will be performed as a part of the Comprehensive Assessment. Depending on the initial assessment, a plan of care will be developed to include specific counseling to patient and family members to be provided by the Social Worker, Chaplain or Bereavement Counselor. The plan of care will reflect family needs, as well as a clear delineation of services to be provided and the frequency of service delivery. Record reviews revealed the date of death for P9 as 12/19/2025. The clinical record failed to reveal a bereavement plan of care. Record reviews revealed the date of death for P10 as 01/19/2026. The clinical record failed to reveal a bereavement plan of care. Record reviews revealed the date of death for P14 as 02/19/2026. The clinical record failed to reveal a bereavement plan of care. On 07/07/2026 at 1:30 PM, an in-office interview was conducted with the Administrator, who revealed that none of the bereavement records contained bereavement care plans. The Administrator further revealed not being aware of the requirement for bereavement plans of care.”

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
