What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Counseling services (42 CFR 418.64(d)(1)(i))
The inspectors’ words, 382 words
“It is required that counseling services include an organized program for providing bereavement services delivered by or under the supervision of a qualified professional with experience or education in grief or loss counseling. Bereavement services are available to the family and other individuals up to one (1) year following the death of the patient. Bereavement counseling extends to residents of a SNF/NF (skilled nursing facility/nursing facility) or ICF/IID (intermediate care facility/Individuals with Intellectual Disabilities) when appropriate and identified in the bereavement plan of care. The bereavement services reflect the needs of the bereaved per a bereavement plan of care that includes: The types of bereavement services to be offered; The frequency of service delivery. This standard was not met as evidenced by clinical record review, document review, and interview. In 2 of 2 (100%) bereavement records reviewed revealed that the bereavement plan of care did not include the frequency of bereavement services. Agency Policy, 2.14 Core Services, Bereavement Services, stated in part, “The Hospice Bereavement Plan of Care is developed and includes services to be provided and the frequency of services delivery. CLINICAL RECORD 13: The patient elected the hospice benefit on 05/10/2024 and died on 08/08/2025. The Bereavement Plan of Care dated 08/11/2025 documented, “Provide bereavement phone calls and visits for at least one year after the patient’s death. Remember the special dates of the patient and family members with cards and/or phone calls”. There was no documented evidence specifying the scope and frequency of the services to be provided. CLINICAL RECORD 14: The patient elected the hospice benefit on 12/23/2024 and died on 01/10/2026. The Bereavement Plan of Care dated 10/07/2025 documented, “Provide bereavement phone calls and visits for at least one year after the patient’s death.” There was no documented evidence specifying the scope and frequency of the services to be provided. During a joint clinical record review and interview on 04/15/2026, the ADON (Personnel Record 4) confirmed that there was no documented evidence specifying the scope and frequency of services to be provided. This was reviewed with the Bereavement Coordinator (BC) (Personnel Record 8) and Administrator (Personnel Record 1) and agreed the evidence was not located in the clinical records. This was reviewed with the BC and Administrator and agreed the evidence was not located in the CRs”
