What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Patient outcome measures (42 CFR 418.54(e)(2))
“Observed in Record Review. In 1 of 16 patient records reviewed, the record of care was not accurate. In the chart for RRNHV#9, the RN, on multiple visits, documented that the patient’s skin was intact, yet also documented that wound care was provided to a lesion on the patient’s leg. It was confirmed with the Administrator that this was not accurate and the patient did not continue to have a wound.”
-
Content of plan of care (42 CFR 418.56(c))
The inspectors’ words, 111 words
“Observed in Record Review. In 2 of 16 patient records reviewed, the plan of care (POC) was not individualized to the time frames required to meet the goals. In the chart for RRHV#4, the goal for the patient to demonstrate the ability to safely use oxygen and care for the equipment was anticipated to be within the benefit period. The timing of the goal was not appropriate to the safety risks associated with the treatment. In the chart for RRNHV#7, the patient developed an open fistula necessitating a drainage bag. The plan of care was not updated with any goals related to this new problem. This was discussed with the Administrator.”
-
Content of plan of care (42 CFR 418.56(c)(2))
“Observed in Record Review. In 2 of 16 patient records reviewed, the plan of care (POC) did not include the scope and frequency of services. In the charts for RRHV#1 and RRHV#2, the volunteer frequency was not included on the POC. This was discussed with the Administrator.”
-
Content of plan of care (42 CFR 418.56(c)(4))
The inspectors’ words, 203 words
“Observed in Record Review. In 5 of 16 patient records reviewed, the hospice plan of care (POC) had did not include all services needed to manage the terminal illness. In the chart for RRHV#4, the oxygen had been increased to 4 liters per minute as confirmed during the home visit and as documented in the IDT notes, yet the POC still read oxygen at 2 liters per minute. In the chart for RRHV#1, the order for the flush of the urinary catheter was incomplete and did not include the volume of the flush nor the indication. In the chart for RRNHV#7, the patient developed an open fistula necessitating a drainage bag. The plan of care was not updated with this problem/intervention. In the chart for RRNHV#13, there were incomplete orders as Humalog and Lantus were both missing the dosage and the foley catheter order was missing the size of the catheter. In the chart for RRNHV#12, the orders were unclear. The patient was NPO with tube feedings, yet many medications were ordered for the oral route including Senna, Zetia, Edavarone, etc. The POC was also unclear if this ALS patient had CPAP, BiPAP or a non-invasive ventilator. This was discussed with the Administrator.”
-
Coordination of services (42 CFR 418.56(e)(4))
The inspectors’ words, 161 words
“Observed in Record Review. In 2 of 16 patient records reviewed, all disciplines that provide care, treatment or services to the patient had not collaborated in the care of the patient. In the chart for RRHV#3, during one home visit, the RN documented the patient’s blood pressure to be 143/110. At other RN visits during the benefit period, the patient’s diastolic blood pressure was noted to be at 80 maximum. There was no documentation that the physician was notified about the change in the patient’s condition. In the chart for RRDC#2, the RN note included discussions with the primary caregiver in two home visits about a probable discharge due to the patient no longer meeting terminally ill criteria. There was no documentation in the record that any other members of the interdisciplinary team (IDT) were aware of the plan to discharge, including notes from the IDT meeting which occurred just prior to the discharge date. This was discussed with the Administrator.”
-
Content (42 CFR 418.104(a)(7))
“Observed in Record Review. In 2 of 16 patient records reviewed, the patient record did not contain an accurate list of medications. In the chart for RRNHV#5, the medication list included Keflex as an active medication from a 7 day course which was ordered more than 3 months prior to the survey. In the chart for RRNHV#6, the RN obtained a urine specimen for a urinalysis. There were no lab results in the patient record. This was discussed with the Administrator.”

- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
