What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Prevention (42 CFR 418.60(a))
The inspectors’ words, 219 words
“It is required that the hospice follows accepted standards of practice to prevent the transmission of infections and communicable disease, including the use of standard precautions, such as hand hygiene. This standard was not met as evidenced by document review, interview and observation. In 1 of 2 (50%) home visit observations revealed the organization did not ensure the staff followed the hand hygiene policy. Agency Policy No. 7-009, Hand Hygiene Policy (revised May 2024), stated in part, “Wet hands and apply the soap, and rub hands together vigorously; avoid use of hot water because repeated exposure to hot water may increase risk of dermatitis. Wash hands for at least fifteen (15) seconds covering all surfaces of the hands and fingers. Rinse with warm water and dry the hands with a disposable towel from the fingers toward the forearm. Use a dry disposable towel to turn off faucet.” HOME VISIT 2: During the home visit, on 01/29/2025, the Nurse Practitioner/Alternate Director of Nursing/Registered Nurse (NP/ADON/RN) (Personnel Record 5), was observed washing hands with soap and water. The NP/ADON/RN turned off the faucet with a forearm (covered with clothing) and then tore off paper towels to dry the hands. During an interview on 01/29/2025, the Director of Patient Care Services agreed that the staff did not follow the hand hygiene policy.”

PERSONNEL RECORD 10: The Certified Home Health Aide’s date of hire was 11/27/2024. The Hospice Aide Competency documented a start date of 11/27/2024.
