What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Patient outcome measures (42 CFR 418.54(e)(2))
The inspectors’ words, 92 words
“Observed in Individual Tracer. A review of 3 of 3 individual tracers, revealed that the organization did not ensure all entries in the medical record were documented accurately. For example, during a review of the informed consent documents for RRHV#1, RRHV#2, and RRHV#3, it was noted that the patient chose the organization’s medical director as the attending physician. However, a review of the certification statement(s) revealed that a different physician signed the document(s) as the attending physician. This observation was reviewed with the Administrator and the Director of Patient Care services.

Content of plan of care (42 CFR 418.56(c))
The inspectors’ words, 104 words
“Observed in Record Review. In 1 of 10 patient records reviewed, it was noted that the organization did not ensure the plan of care goals included outcome measures for specific care activities, For example, during a review of the medical record for RRNHV#3, the comprehensive assessment included a description of a stage 2 pressure ulcer at the sacral area. The plan of care did not include interventions related to wound care and a goal statement to evaluate the response to care, treatment, and services related to wound care interventions. This observation was reviewed with the Administrator and the Director of Patient Care Services.

The Hospice Aide performed visits 3 times per week starting the 6th week of services during the first benefit period. Hospice Aide visits continued to be performed 3 times per week during the 2nd benefit period.

Example #2: During a review of the medical record for RRNHV#5, the plan of care included an order for Hospice Aide visits 2 times per week.
