The short answer

Record last released by CMS on Oct 23, 2026. We couldn’t read Centers for Medicare & Medicaid Services’ list since Oct 27, 2026: qcor.cms.gov now refuses our reader. This record is as of then.

The last 24 months 6 deficiencies in 1 month of the last 36. Last cited Jul 2026. Most serious: Standard-level. Every month

What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Jul 16, 2026: reaccreditation inspection by The Joint Commission6 deficiencies
  • Plan Based on All Hazards Risk Assessment (42 CFR 418.113(a)(1))

“Observed in Emergency Management Session. The organization’s community-based risk assessment did not utilize an all-hazards approach. For example: The hazard vulnerability analysis did not include the following: Technological hazards (such as utility or information technology outages) and emerging infectious diseases (such as Ebola, Zika Virus, SARS-CoV-2). This was confirmed by the Administrator.”

    Rule E0006. Standard-level.

  • Content of plan of care (42 CFR 418.56(c))

“Observed in Tracer Visit. In 4 of 8 records reviewed, the plan of care did not include goals that addressed all identified interventions. Specifically, in RRHV#1, RRHV#2, RRNHV#3, and RRDC#2, oxygen-related interventions were included in the care plan. However, individualized and measurable goals specific to the patients’ oxygen needs were not developed. This finding was confirmed by the Director of Nursing (DON).”

    Rule L0545. Standard-level.

  • Content of plan of care (42 CFR 418.56(c)(1))

    The inspectors’ words, 110 words

“Observed in Tracer Visit. In 1 of 8 records reviewed, the plan of care contained oxygen range orders without clearly defined parameters or indications for adjusting the oxygen flow rate. For example, in RRHV#3, the plan of care included an oxygen order of 1-5 LPM PRN. However, specific clinical parameters and indications for titrating the oxygen settings were not identified. This finding was confirmed by the Director of Nursing (DON). Observed in Tracer Visit. In 2 of 8 records reviewed, oxygen orders did not specify the frequency of administration. Specifically, in RRHV#1 and RRHV#2, the oxygen orders lacked frequency instructions. This finding was confirmed by the Director of Nursing (DON).”

    Rule L0546. Standard-level.

  • Coordination of services (42 CFR 418.56(e)(2))

“Observed in Tracer Visit. In 1 of 3 home visits, the organization did not provide services according to the patient’s individualized plan of care. For example: In RRHV#1, oxygen was ordered on the plan of care but was not present in the patient’s home at the time of the Hospice Surveyor’s home visit. Per the patient’s wife, the oxygen had never been delivered since admission six weeks ago. This was confirmed by the DON.”

    Rule L0555. Standard-level.

  • Supervision of hospice aides (42 CFR 418.76(h)(2))

“Observed in Tracer Visit. In 1 out of 9 HR files, the organization did not conduct an annual on-site aide visit to the location where a patient was receiving care in order to observe and assess the aide while the aide was providing care. For example: In HR#6, there was no evidence of an annual on-site aide visit in the Aide’s HR file. This was confirmed by the DON.”

    Rule L0632. Standard-level.

  • Training (42 CFR 418.100(g)(3))

“Observed in HR File Review. In 1 of the 9 HR records reviewed, the organization was unable to provide documentation of defined competencies for the role of Volunteer Coordinator. This was confirmed by the HR Director.”

    Rule L0663. Standard-level.

Oct 27, 2023: follow-up inspection by The Joint Commission1 deficiency
  • Review of the plan of care (42 CFR 418.56(d ))

“Observed in Tracer Visit. In 1 of 1 active patient records reviewed, the Interdisciplinary Group did not review, revise and documents the plan as frequently as every 15 calendar days. Identified during the record review for the only active patient, the IDG conducted two weeks prior to the survey date was conducted on day 16. This was reviewed and validated with the Director of Nursing.”

    Rule L0552. Standard-level.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
