What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“Observed in Record Review. In 1 of 18 patient records reviewed, it was noted that the patient record was incomplete. For example, in record RRHV#4 the documentation of the foley catheter RN changed catheter and placed 16fr foley with 10ml balloon did not include technique used, what the balloon was inflated with or how much was placed in the balloon. This was confirmed and discussed with the DPCS and CCO present.”
-

The inspectors’ words, 107 words
“Observed in Record Review. In 2 of 18 patient records reviewed, it was noted that the plan of care did not include all treatments necessary to meet the patient’s needs. It stated: Foley catheter changed 16 French secured with 10ml balloon with no frequency, technique or what balloon was to be secured with. -
Coordination of services (42 CFR 418.56(e)(2))
The inspectors’ words, 164 words
“Observed in Record Review. In 3 of 18 patient records reviewed, it was noted that the organization had not provided patient care according to the patient’s individualized plan of care. For example, in record RRHV#2 the volunteer did not follow the visit frequency of 3-4 visits per month of 1 hour each. The volunteer had stopped seeing the patient, however the POC continued to have the volunteer frequency order on it for 3 IDGs. In record RRHV#6 the volunteer plan frequency stated ‘2-4 visits/mnth for 2-4 hours each’ was not included on the IDG or a supplemental order. There were goals and intervention on IDG but no frequency and the volunteer had completed visits. In record RRNHV#6 the volunteer plan frequency stated 1 visit a week was not included on the IDG or a supplemental order. Counseling services (42 CFR 418.64(d)(1)(iv))
“Observed in Record Review. In 3 of 3 bereavement records reviewed, , the Hospice pharmacy surveyor noted that the organization had not individualized the bereavement care plans as to when a particular goal identified by the bereavement coordinator post death would actually be achieved or planned to be achieved For example, the goals identified for each bereaved individual were left blank containing no plan to reach aby of the goals The lack of individualization was validated with organization leadership at the time of the survey,”
-
Training (42 CFR 418.78(a))
“Observed in HR File Review. In 2 of 2 HR files reviewed, the Hospice pharmacist noted that neither of the appropriate volunteer files reviewed included documentation of orientation and training consistent with Hospice industry standards. This omission was validated with the Director of Volunteer Services and pharmacy leadership at the time of the survey.”
-
Criminal background checks (42 CFR 418.114(d)(1))
“Observed in HR File Review. During a review of the file for the contracted medical director, the Hospice pharmacy surveyor noted that the physician’s contract was signed on 6/1/24 and on that day he had access to patient records. His background check was scheduled by the organization approximately one year after his contract began. In addition to the Hospice requirement, the organization required a background check for all employees prior to starting employment, This omission was validated with the HR manager and organization leadership at the time of the survey.”
