The short answer

Record last released by CMS on Oct 23, 2026. We couldn’t read Centers for Medicare & Medicaid Services’ list since Oct 27, 2026: qcor.cms.gov now refuses our reader. This record is as of then.

The last 24 months 5 deficiencies in 1 month of the last 36. Last cited Oct 2024. Most serious: Standard-level. Every month

What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Content of comprehensive assessment (42 CFR 418.54(c)(6)(i))

    The inspectors’ words, 269 words

“It is required that the documented comprehensive assessment includes a drug profile that contains the patient’s current prescription and over the counter (OTC) drugs, supplements, herbal remedies, other alternative treatments that could affect drug therapy. The medication review process includes the identification of the following, the effectiveness of drug therapy, drug side effects, actual or potential drug interactions, duplicate drug therapy, drug therapy associated with laboratory monitoring. The assessment includes evidence that common side effects of medication in the hospice populations are anticipated, and as appropriate, preventive measurements are implemented to manage the side effects. The drug profile is updated per the hospice policy and procedure, but, at minimum, at the time of each comprehensive assessment and/or when new medication is added, or changes are made to existing medication. This standard was not met as evidenced by clinical record review and interview. In 1 of 3 (33%) home visit observations, the organization did not ensure a medication review was completed, and the medication profile was updated. Home Visit 3: The patient elected the hospice benefit on 09/16/2024. During the home visit observation with the Hospice Aide (HA) (Personnel Record 9), a medication review was completed by comparing the patient’s home medications to the hospice medication profile. The patient’s home medications included Aripiprazole 2mg po [by mouth] daily started on 09/03/2024 per the facility’s administrator. The hospice medication profile did not include Aripiprazole. During a joint record review and interview on 10/10/2024, the Regional Director/ Registered Nurse/ Bereavement Coordinator/ Volunteer Coordinator confirmed the medication review had not included Aripiprazole, and the medication was not included in the patient’s medication profile.”

“It is required that the hospice documents the cost savings achieved through volunteers. Documentation includes the identification of each position that is occupied by a volunteer, the work time spent by volunteers occupying those positions, and estimates of the dollar costs that the hospice would have incurred if paid employees occupied the positions noted in Item 1 and for the durations noted in Item 2. During an interview on 10/10/2024, the Regional Director/ Registered Nurse/ Bereavement Coordinator/ Volunteer Coordinator confirmed the agency did not have documentation of volunteer hours from January 2024 through September 2024. Upon reviewing the volunteer program, the documentation lacked evidence of the number of volunteer hours, the volunteer percentage, and the cost savings the hospice would have incurred if paid employees occupied the positions from January 2024 through September 2024. There were no documented volunteer hours for 2024.”

    Rule L0646. Standard-level.

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
