What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Content of plan of care (42 CFR 418.56(c)(1))
The inspectors’ words, 170 words
“Observed in Record Review. In 1 of 10 records reviewed, the surveyor noted that the organization failed to ensure that medication orders or prescriptions are clear and accurate. For example, in HOSRRHV#1, the patient had two as needed pain medications ordered (Hydrocodone/Acetaminophen q 4 hours PRN for pain/headache, and Acetaminophen q 6 hours PRN for headache), however, there was no indication as to which medication should be taken first. This was reviewed with the Director of Nursing and QAPI Director. Observed in Record Review. In 1 of 10 records reviewed, the surveyor noted that the organization failed to create a plan of care for the patient that includes all services needed for the palliation and management of the terminal illness and related conditions. For example, for HOSRRHV#2, the nurse identified the problems of UTI and impaired urinary elimination with current Foley catheter placement, however, there were no care plans detailing interventions and goals for the management of these problems. This was reviewed with the Director of Nursing and QAPI Director.”
-
Prevention (42 CFR 418.60(a))
The inspectors’ words, 105 words
“Observed in Patient Home. In 1 of 3 home visits, the surveyor noted that the organization failed to implement activities for the prevention and control of infections and communicable diseases. For example, in HOSHV#1, the Aide did not follow the organization’s policy for hand hygiene. During the course of providing a bath to the patient, the aide stopped bathing patient to retrieve a towel from the closet in the hallway outside of patient’s room. Before resuming care, the aide removed gloves and donned a new pair of gloves without sanitizing hands, then proceeded to bathe the patient. This was reviewed with the Director of Nursing.”
-
Training (42 CFR 418.100(g)(3))
“Observed in Competency Session. During the Competence Assessment/HR File Review session, the surveyor noted that the organization failed to ensure that staff are competent to perform their responsibilities. For example, in HR#3 (Medical Director), HR#4 (Alternate Medical Director) did not have a competency assessment performed every 3 years per the organization’s policy, and HR#6 (Chaplain) did not have a competency assessment performed upon hire per the organization’s policy. This was reviewed with the Director of Nursing and The QAPI Director.”

- CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
- Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.
