The short answer

Record last released by CMS on Oct 23, 2026. We couldn’t read Centers for Medicare & Medicaid Services’ list since Oct 27, 2026: qcor.cms.gov now refuses our reader. This record is as of then.

The last 24 months 3 deficiencies in 1 month of the last 36. Last cited Feb 2026. Most serious: Standard-level. Every month

What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Hospice aide assignments and duties (42 CFR 418.76(g)(1))

    The inspectors’ words, 302 words

“It is required that written patient care instructions for a hospice aide are prepared by a RN who is responsible for the supervision of the hospice aide. This standard was not met as evidenced by clinical record review and interview. In 2 of 15 (13%) clinical records reviewed in which hospice aide services were accepted revealed the agency did not ensure the registered nurse provided specific written instructions for the hospice aide. CLINICAL RECORD 15: The patient elected the Hospice benefit on 10/16/2025 with documented diagnosis of Senile degeneration of the brain. The hospice aide care plan dated 11/12/2025 included the following relevant tasks: “Bathing (bed); bathing (shower); dressing; hair care; HOB (head of bed) elevated; nail care-file or trim fingernails or toenails, oral care; position for comfort; shampoo; skin care; change linens; RN communication for incomplete tasks; fall precautions.” All tasks were to be completed every visit. Aide frequency of three times weekly. The aide care plan did not provide the aide(s) with specific instructions for which type of bath to provide for the patient during the visit(s). CLINICAL RECORD 17: The hospice aide care plan dated 05/01/2025 included the following relevant tasks: “Bathing (bed); OR bathing (shower); routine ostomy care, stand by assistance with ambulation; fall precautions.” All tasks were to be completed every visit. Aide visit frequency of twice weekly. The aide care plan did not provide the aide(s) with specific instructions for which type of bath to provide for the patient during the visit(s). A joint clinical record review and interview was conducted with the Director of Clinical Services on 02/11/2026 during which time she advised “they had been conducting audits of the aide care plans to modify the ones that had two types of baths, adding oxygen and fall precautions and clarifying other instructions for the aides.””

    Rule L0625. Standard-level.

  • Hospice aide assignments and duties (42 CFR 418.76(g)(2)(ii))

    The inspectors’ words, 185 words

“It is required that a hospice aide provides services included in the plan of care. This standard was not met as evidenced by clinical record review and interview. In 1 of 15 (7%) clinical records reviewed in which hospice aide services were accepted revealed the agency did not ensure hospice aide provided services included in the plan of care. CLINICAL RECORD 3: The hospice aide care plan dated 02/06/2026 included the following relevant tasks: “Assist to bedside commode; bathing (shower); dressing; hair care; perineal care; shampoo; skin care; transfer to/from bed/chair; empty trash; RN communication for incomplete tasks.” All tasks were to be completed every visit. The aide visit dated 02/09/2026 noted the following: “Bathing (shower) No. Gave bed bath.” The aide did not follow the plan of care and documented they gave a bed bath instead of a shower. A joint clinical record review and interview was conducted with the Director of Clinical Services on 02/11/2026 during which time she advised “the aides should call a registered nurse to have the care plan adjusted if they are not providing the type of bath listed.””

    Rule L0626. Standard-level.

  • Hospice aide assignments and duties (42 CFR 418.76(g)(4))

    The inspectors’ words, 205 words

“It is required that Hospice aides must report changes in the patient’s medical, nursing, rehabilitative, and/or social needs to a registered nurse as the changes relate to the plan of care and any quality assessment and improvement activities. This standard was not met as evidenced by clinical record review and interview. In 1 of 15 (7%) clinical records reviewed in which hospice aide services were accepted revealed the agency did not ensure that the hospice aide reported changes to a registered nurse (RN). CLINICAL RECORD 3: The hospice aide care plan dated 02/06/2026 included the following relevant tasks: “Assist to bedside commode; bathing (shower); dressing; hair care; perineal care; shampoo; skin care; transfer to/from bed/chair; empty trash; RN communication for incomplete tasks.” All tasks were to be completed every visit. The aide visit dated 02/09/2026 documented: “Bathing (shower) No. Gave bed bath.” The clinical record did not provide evidence that the Hospice Aide reported the changes to the care plan to the RN. A joint clinical record review and interview was conducted with the Director of Clinical Services on 02/11/2026, who stated, “The aides should call a registered nurse to have the care plan adjusted if they are not providing the type of bath listed.””

What the record doesn’t say

  • CMS doesn’t publish fines or other penalties for hospices, so none are shown.
  • Only complaints that led to a citation appear; complaints that didn’t aren’t published.
  • CMS ended its Special Focus Program for hospices with serious problems on Feb 14, 2025, so no hospice is marked as one.
  • Hospice quality measures have a gap from October 2025: CMS replaced its patient assessment then, and public results from the new one (HOPE) are expected around November 2027.

The public record above comes first. Anything the hospice adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the hospice will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the hospice”, and paying will never change a finding, a rating or the order of results.
