What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

At Discharge
The inspectors’ words, 203 words
“At discharge CFR(s): 484.55(d)(3) At discharge. This Element is not met as evidenced by: Based on record reviews, policy review, and staff interview, it was determined that the agency failed to ensure that a comprehensive discharge summary was completed to include the administration of the Outcome and Assessment Information Set (OASIS) at discharge to assist in the continuity of care post discharge for one patient (P) (P#6) of two sampled discharge patients. Findings include: Review of the policy titled, “Discharge Summary”, effective date 9/5/22, indicated the discharge summary will incorporate findings from the discharge OASIS assessment. Record review revealed P#6 was admitted to services on 3/13/2023 with a diagnosis of Non-pressure chronic ulcer unspecified part of lower leg (open sore on the skin) and discharged from the agency on 5/25/2023. Review of the document titled, “Discharge Summary” dated 5/25/2023, revealed that the summary lacked the OASIS data assessment which must be completed at time of discharge. On 12/5/2023 at 1:00 p.m., an interview was conducted at the agency with the Administrator who confirmed the discharge OASIS was not completed at discharge. The Administrator did confirm the nurses complete the discharge summaries but were not aware that OASIS discharge assessments were required as well.”

How do patients fare?
- CMS gives no CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.: “The number of patient episodes for this measure is too small to report.”

What the record doesn’t say
- CMS doesn’t publish fines or other penalties for home health agencies, so none are shown.
- Only complaints that led to a citation appear; complaints that didn’t aren’t published.
- QCOR says some of its home health reports are accurate or available only through May 19, 2021, because of a systems migration.
- Many home health agencies are inspected by a private accrediting organisation instead of the state, and QCOR’s home health file doesn’t appear to include those inspections: no inspection on record here doesn’t mean none took place.
