What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Ic - Hbv - Test Pts/Staff Post Last Dose

    The inspectors’ words, 287 words

“IC-HBV-TEST PTS/STAFF POST LAST DOSE CFR(s): 494.30(a)(1)(i) Hepatitis B Screening: Patients and Staff Test all vaccines [patients and staff] for anti-HBs 1-2 months after last primary vaccine dose. -- If anti-HBs is <10 mIU/mL, consider patient or staff member susceptible, revaccinate with an additional three doses, and retest for anti-HBs. -- If anti-HBs are =10 mIU/mL, consider immune, and retest patients annually. -- Give booster dose of vaccine to patients if anti-HBs declines to <10 mIU/mL and continue to retest patients annually. This Standard is not met as evidenced by: 46466 Based on interviews and facility document review, the facility staff failed to administer tuberculosis screenings for one (1) of six (6) patient records sampled. The findings include: On 3/18/24 at 3:05 p.m., a review of Patient #4’s (Pt.#4) medical record was conducted. The record documented that Pt.#4 had a “sensitivity” to the purified protein derivative (PPD). PPD is used to diagnose a person for latent tuberculosis infection (LTBI). Pt.#4’s record also listed Pt.#4 as a “reactor” to PPD. Pt.#4 received a chest x-ray to screen for LTBI on 5/27/22, prior to Pt.#4’s admission to the facility on 6/10/22. There was no documented evidence that Pt.#4 received a tuberculosis (TB) symptoms screening in 2023. On 3/18/23 at 3:20 p.m., Staff Member #18 confirmed that the 2023 TB screening documentation was not present in Pt.#4’s record and that it should’ve been completed and documented. A review of the facility’s policy “Tuberculosis Infection Control Policy”, last revised 4/2018, stated in part: “...6. A TB-Risk Appraisal Questionnaire (TB-RAQ) (attached) will be administered by a licensed nurse to patients on admission and annually thereafter...”. The findings were discussed with Staff Members #1, #8, #10 and #18 on 3/19/23 at 12:15 p.m.”
