What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Pe - Safe, Functional, Comfortable Environment

    The inspectors’ words, 211 words

“PE-SAFE/FUNCTIONAL/COMFORTABLE ENVIRONMENT CFR(s): 494.60 The dialysis facility must be designed, constructed, equipped, and maintained to provide dialysis patients, staff, and the public a safe, functional, and comfortable treatment environment. This Standard is not met as evidenced by: 16136 Based on observation, document review, and interview, the facility does not ensure the pathway of egress is unobstructed, fire doors are not propped open, and fire doors are inspected annually. Findings include: Observation on 05/06/25 and 05/07/25 revealed all the fire rated doors throughout the facility were propped open with a rubber wedge for nine of nine doors. Observation on 05/07/25 at 01:10 PM revealed the fire door in the marked egress, adjacent to the staff breakroom, was locked and required a passcode to open the door. Interview with Staff (E), Area Technology Operations Manager and Staff (H), Biomedical Technician, did not have knowledge if the locking mechanism released upon activation of the fire alarm system. From 05/06/25 to 05/07/25, the facility failed to provide requested documentation to verify the fire doors had been inspected annually by a qualified person and/or provide a written record of the inspection per National Fire Protection Association (NFPA) 80-2010 edition. An interview on 05/07/25 at 02:30 PM with Staff (E), Area Technology Operations Manager, verified these findings.”

    Rule V0401. Standard-level.

  • Provide Ordered Supplies/Equipment

    The inspectors’ words, 240 words

“H-PROVIDE ORDERED SUPPLIES/EQUIPMENT CFR(s): 494.100(c)(1)(vi) Services include, but are not limited to, the following: (vi) Purchasing, leasing, renting, delivering, installing, repairing and maintaining medically necessary home dialysis supplies and equipment (including supportive equipment) prescribed by the attending physician. This Standard is not met as evidenced by: 16136 Based on document review, medical record review, and interview, for three of three patients reviewed, the facility does not ensure that the maintenance on the home dialysis equipment is performed (Patient #12, 13 and 14). Findings include: Review of the manufacturer’s recommendations for the NxStage hemodialysis machine indicate the blood leak detector must be cleaned monthly, the drain line must be flushed and disinfected a minimum of monthly, the air filter must be cleaned or replaced quarterly, the prefilter must be replaced annually, and the sediment filter must be replaced annually. Review of the electronic medical records for Patient’s #12, #13 and #14 revealed no evidence that the routine maintenance had been performed on NxStage hemodialysis machine (home therapies for hemodialysis, a process that filters blood outside the body to remove waste and excess fluid) per manufacturer’s recommendations. Specifically, there was no evidence for quarterly air filter maintenance for all three patients, that the annual sediment filter had been replaced for Patient’s #13 and Patient #14, and that the monthly blood leak detector was cleaned monthly for Patient #12. Interview on 05/07/25 at 02:20 PM with Staff (F), Registered Nurse, verified these findings.”

    Rule V0597. Standard-level.
