What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Fac Receive/Review Self Monitoring Data Every 2 Months
The inspectors’ words, 419 words
“H-FAC RECEIVE/REVIEW PT RECORDS Q 2 MONTHS CFR(s): 494.100(b)(2),(3) The dialysis facility must - (2) Retrieve and review complete self-monitoring data and other information from self-care patients or their designated caregiver(s) at least every 2 months; and (3) Maintain this information in the patient ’ s medical record. This Standard is not met as evidenced by: 32003 Based on medical record review, training handout review, and staff interviews, the facility staff failed to ensure a patient performed treatments as prescribed and failed to ensure the SAK and PAK lot numbers were documented on the treatment sheets for 1 of 2 HHD patient (Patient #4). The findings include: Open medical record review on September 26, 2024 revealed, Patient #4 was admitted to the HHD program on December 20, 2021. The patient completed HHD solo training on February 10, 2022. Order review revealed, the patient was prescribed four treatments per week. Treatment sheet review from September 9, 2024 through September 20, 2024 (8 treatments) revealed, 6 of 8 treatments were performed and the treatment days were September 11th, 12th, 14th, 16th, 17th, and 19th, 2024. Additionally, the SAK and PAK lot number were not entered for each treatment day. Review revealed, the training handout did not include any verbiage about performing treatments as prescribed or about entering the SAK and PAK lot numbers for each treatment. Interview on September 26, 2024 at 1:00 PM with the Area Manager for Home Therapy revealed, the manager spoke with the HHD machine representative and was informed that if the patient never entered the SAK and PAK lot numbers since starting treatment, then that section would not show in the [electronic system] and would be blank on the treatment sheet. The manager continued with that the patient received face-to-face, step-by-step instructions via “Patient Portal” on the iPad and with staff present. The electronic education emphasized that the SAK and PAK lot number entries were required information because it could become a safety issue especially during instances when the chloramine testing did not pass. Interview revealed, the manager planned to follow up with Clinical Services because of the inability to find a policy that spoke to the electronic entry requirements and because the electronic system did not provide any alerts when treatment sheet information was missing. Interview on September 26, 2024 at 1:45 PM with Nurse #1 revealed, two nurses provided care and support to HHD patients, carried a patient case load of up to 38-patients, and treatment sheets were reviewed daily, monthly, or as needed.”
